An opportunity has arisen for an enthusiastic, compassionate and dynamic Band 7 District Nurse to join our District Nursing team in Milton Keynes leading a team of Registered Nurses, Nurse Associates and Health Care Support Workers.

Main duties of the job
The District Nurse will be responsible for a caseload of patients including case management of our complex and end of life patients. The District Nurse will be highly skilled, professional and able to provide holistic assessments and develop comprehensive nursing plans of care to meet the needs of each patient on caseload. They will also work in partnership with primary health care teams, social care, voluntary sectors and other CNWL services ensuring that the patient is involved and at the centre of all care provided.

Hours of service are 09:00 to 17:00, to include weekends and bank holidays on a rotational basis and being a car driver is essential.
