We have a Health and Justice Services Directorate, within which sits the 'Children and Young People' Service line working in partnership with 'Youth Custody Service' colleagues to provide trauma-informed primary care, mental health, neuro-developmental and substance misuse services to the young people and young adults within our care.

We are looking to recruit motivated, innovative and enthusiastic Band 5 Nurses to join our Integrated Healthcare Service at HMP YOI Feltham. Feltham is effectively two separate establishments delivering healthcare services to young people in Feltham A (15-19 years) and young adults in Feltham B (18-25 years). These two separate cohorts have slightly different needs and we are looking for staff that understand their needs and work in line with 'Child First' principles. By joining our team, you will not only look after the healthcare needs of the young people/young adults we care for, but also have the opportunity to have a real impact on promoting health and well-being for this population.
Main duties of the job
The post holder will be Registered Adult or Children's Nurse, working as part of an excellent primary care team, carrying out a range of clinical activities in a secure environment where the young person/young adult is put first.
The post holder will often be the first point of contact for the healthcare needs through initial assessment, screening and/or triage of young people/young adults in accordance with specific standard operating procedures. The successful candidate will assess, plan and implement care for patients within each care/treatment pathway in order to promote wellbeing, facilitating the use of evidence-based practice to ensure clinical care is both effective and appropriate to the young person/young adult.
The post holder will carry out general nursing duties, maintain contemporaneous records, support their colleagues and make a professional contribution to the development of an integrated healthcare service.
The post holder will also act as a preceptor/mentor according to experience.

Clinical
Assess, plan, implement and evaluate care for patients using agreed Patient Group Directives orcare pathways where appropriate. Plans and organises own workload without direct supervision in order to meet the requirementsof the regime. Administer medication as prescribed, which may include oral, immunisations, intramuscularinjections, insulin or nebulisers, and may be compounded by frequent unplanned interruptionssuch as other demanding patients, staff requests. Treat where possible (e.g. Hep B and Flu vaccines, minor ailments), referring on to otherdepartments, clinical specialists or any other appropriate person as required. Ensure that those identified as requiring specialist input such as mental health or substancemisuse, are referred in as timely a manner as possible and that relevant paperwork is completedand care pathway communicated to relevant staff, taking due account of the sensitivity of anyinformation shared with non healthcare professionals who have to manage the men on thewings. For those identified as being at risk of self-harm, complete an ACCT document following athorough assessment and communicate concerns to the appropriate member of staff. Provide appropriate and timely health promotion interventions to the men and advise staffaccordingly. Participate in health promotion/harm minimisation clinics in specific disease related areas,ensuring that clinics run smoothly and meet national and local priorities and guidelines e.g. Smoking cessation Hepatitis B Sexual health Diabetes Asthma Coronary Heart Disease Will be required to take, collect or transport specimens such as blood, wound swabs or urine. Will be required to undertake wound dressings. Ensure that men being discharged back to the community have sufficient medication and adischarge letter summarising their care package for their GP.
