Job summary

The purpose of the role is to oversee and undertake physical health checks and health promotion support to complex patients with severe and enduring mental health problems, (SMI) who are under the care of Brent Community mental health teams (CMHT); Inpatient services, Learning disability services (LD), Home treatment team (HTT), Older adults (OPHA) and Early intervention services (EIS). The postholder will supervise and support the teams to deliver high standards of physical care to patients with long term conditions in accordance with up to date, evidence-based professional practice subject to local and Trust policies and to provide leadership to the nursing teams within the services. They will focus on supporting patients to be healthy, monitoring of long-term conditions, health promotion and screening.

The post holder will lead on preparation and delivery of health promotion initiatives linking in with Trust resources, local business, and healthcare providers. The post holder will ensure that health promotion materials are up to date, available in various languages and easily accessible across the services. Based on the principles of co-production, the post holder will support the administration and facilitation of service user forums and feedback relating to physical health and health promotion initiatives

Main duties of the job

  • Provide professional clinical leadership, working in collaboration with the appropriate multidisciplinary groups to meet the physical health needs of patients within community services.
  • Provide specialist evidenced based knowledge and clinical skills to the staff and patients within the services to ensure that the physical health and well-being needs of individuals is met.
  • Lead on the strategic development and delivery of health promotion and physical health care within CMHT, OPHA, HTT, EIS and Acute Inpatient Services.
  • Active involvement and leadership on our QI Project on physical health Improvement model
  • Expected to complete QI training to assist with QI project
  • Working collaboratively with medical members of the team to deliver high standards of physical health care.
  • To continually strive to improve physical health care of service users through our services.
  • Phlebotomy trained; including training on how understanding blood results, or expected to complete training and support doctors in carrying out blood tests.
  • Develop and deliver training on physical health and health promotion to clinicians within the services as required.
  • Develop and maintain good links with other services, such as occupational therapy and primary care as required.
  • Work collaboratively with the Physical Health Champions within Borough, Physical Health Leads and Nursing Directors in Jameson.
