Job summary
The Early Stroke Rehabilitation Team (ESRT) is a friendly, enthusiastic and supportive team who are passionate about delivering high quality patient-centred care.
We have an exciting opportunity for a band 6 physiotherapist to join the team on a fixed-term basis (12 months). You will work alongside a team of physiotherapists, occupational therapists , speech and language therapists, clinical neuropsychologist, stroke nurse and rehabilitation assistants.
The ESRT is based in Bletchley, Milton Keynes, you will be working in the community as part of the team providing holistic, goal orientated rehabilitation for people who have a new stroke.

Main duties of the job

Working as part of a multi disciplinary team
- Undertaking assessment and treatment of patients with stroke in a community setting
- Plan and implement a comprehensive programme of intervention for patients with stroke, including those with a complex presentation/multi-pathologies

Work alongside our partners across the stroke pathway for example the Stroke Clinical Specialist, other community services, and the acute hospitals

To work collaboratively as part of an inter-disciplinary approach within ESRT and Neuro Rehab Service by attending and contributing to multidisciplinary meetings, participation in patient discussions/treatment plans and discharge planning.
