Job summary

The Early Stroke Rehabilitation Team (ESRT) is a friendly, enthusiastic and supportive team who are passionate about delivering high quality patient-centred care.

We have an exciting opportunity for a band 6 physiotherapist to join the team on a fixed-term basis (12 months). You will work alongside a team of physiotherapists, occupational therapists , speech and language therapists, clinical neuropsychologist, stroke nurse and rehabilitation assistants.

The ESRT is based in Bletchley, Milton Keynes, you will be working in the community as part of the team providing holistic, goal orientated rehabilitation for people who have a new stroke.

Main duties of the job

  • Working as part of a multi disciplinary team

  • Undertaking assessment and treatment of patients with stroke in a community setting
  • Plan and implement a comprehensive programme of intervention for patients with stroke, including those with a complex presentation/multi-pathologies
  • Work alongside our partners across the stroke pathway for example the Stroke Clinical Specialist, other community services, and the acute hospitals

  • To implement strategies to empower patients and families/carers to take a lead role in their rehabilitation and long term management of their condition

  • To work collaboratively as part of an inter-disciplinary approach within ESRT and Neuro Rehab Service and Neuro Clinical Specialist Team by attending and contributing to multidisciplinary meetings, participation in patient discussions/treatment plans and discharge planning

  • oFull & current UK driving licence and access to a suitable vehicle.

Education & Qualifications

  • oUK registration with Health and Care Professions Council (HCPC)
  • oPhysiotherapy degree or equivalent Physiotherapy professional qualification

Desirable

  • oEvidence of post registration CPD relevant to Neurology/ Stroke
  • oMembership of ACPIN or other relevant specialist interest group

Previous experience

Essential

  • oBroad base experience working as a physiotherapist in the NHS in a community or acute setting
  • oExperience working within neurological conditions and/or stroke in a community or acute setting
  • oExperience of working effectively in a multi-disciplinary team
  • oAbility to plan, prioritise and manage own workload effectively

Desirable

  • oUndertaken or participated in a service delivery project, quality improvement project or audit
  • oExperience of being a student clinical educator
