Home 1st Twilight Nursing service is looking to recruit colleagues who are patient focused, dynamic and have the ability to work autonomously.
The Home 1st Twilight Nursing service provides out of hours unplanned nursing care to patients in their own homes. The service operates between the hours of 7pm and 2am.
Main duties of the job
The successful candidate will:
Have developed a wide range of practical nursing skills which are demonstrated to a high standard.
Work with the Home 1st Nursing service to provide skilled patient care to patients and their families in their own homes or other settings as appropriate.
Provide skilled nursing intervention/care to patients with a range of conditions.
Work independently and without direct supervision and have the ability to prioritise a changing workload.
Ensure clear, concise and accurate records are kept and updated in a timely manner, including the use of departmental electronic records.
To be aware of health and safety at work and follow risk assessments to ensure the safety of service users and staff.
Apply knowledge/skills gained from education/training so that a high standard of care is maintained at all times.
Ensure the needs of carers and relatives are listened to and reported back to the Nurse in Charge of the shift.

As a member of the Home 1st Twilight Nursing Service
- Provides unplanned nursing care to patients in Milton Keynes between the hours of 8pm and 1am.
- Provides skilled nursing intervention/care to patients with a range of conditions in partnership with the Home 1st Nursing Service and the clinical direction of the General Practitioners within the management structure of the employing Trust.
- Maintains close working relationships with the Home 1st Services, District Nursing Service, GP services and other members of the multi-disciplinary teams involved in the patients care.
- Applies current Infection Control Policy
- Encourages patients towards self-care and independence
- Demonstrates, teaches and supervises the nursing care that families/carers can safely undertake
- Ensures that adequate medical supplies, equipment and other materials are maintained for patients
- To obtain consent to any procedure from each patient and to proceed only when consent is obtained. Document when procedures are carried out in patients best interests.
- Be competent with the steps to be taken in medical emergencies and act accordingly.
- Maintain contemporaneous records that are recorded in a timely manner.
