Job summary

A wonderful opportunity has opened up for an aspiring RMN to join our healthy ageing community mental health service at the heart of Harrow, where you will be afforded the time and space to engage inspecialised training and developmentopportunities to help you become a more enhanced community practitioner. We are looking for experienced, focused individuals, who are highly motivated with excellent communication and managerial skills. Community Services are becoming fast paced and rapidly changing to support the 'Out of Hospital Care' strategies, you will therefore need to demonstrate resilience in the ability to adapt to the changing landscape and opportunities as the service develops. Main duties of the job

You will be expected to triage referrals and allocate work to junior staff as appropriate in addition to providing a comprehensive mental health assessment for complex patients in the acute phase of a mental health crisis.

You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services and the HTT

The CMHT is a multidisciplinary team, comprising of CPNs, psychologists, Art therapist, social workers, OT and doctors. The team is supported by a good admin system and offers excellent work environment. The team has good links with local statutory and non-statutory services. We are based in the Wealdstone area of Harrow, which has easy links to Central London and good local transport connections. The team works closely with Ellington Ward (Bedded unit) based in Northwick Park Hospital.

The HTT is a nurse led service with a dedicated medic and is well embedded in the CMHT.

  • To triage referrals and allocate work to junior staff as appropriate.
  • To provide a comprehensive mental health assessment for complex patients in the acute phase of a mental health crisis.
  • To assess current risks and identify how the risk and patients mental health needs would best be met.
  • To formulate, implement and review individualised Recovery Care Plans with patients and their carers that aims to address their psychological, physical & social needs.
  • To build and maintain therapeutic relationships with patients and carers and involve them in drawing up care plans.
  • To work with patients care co-ordinator/ lead professional as appropriate to ensure continuity of care.
  • To provide information and advice to carers to support them through the crisis period.
  • To administer , supervise and monitor prescribed medication advising patients and carers on storage, administration and possible side effects.
  • To have a sound understanding and knowledge of current legislation and itsapplication to service users with mental health problems e.g. Mental Health Act, Mental Capacity Act, Human Rights Act, Freedom of Information Act, Data Protection Act.
