Job summary

An exciting opportunity has arisen to work for the South Kensington & Chelsea Mental Health Hub (CMHH).

Based in Fulham Road, Chelsea, the South K & C CMHH is looking for a passionateMental Health worker who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities.

There are two Community Mental Health Hubs in Kensington & Chelsea, one North and one in the South, each is aligned to a local Primary Care Networks (PCN). Working in this way allows each Hub to have a core structure and function to meet the needs of the local population, aligned with our primary care colleagues.There is good interaction between our hubs, fostering a supportive and friendly atmosphere of mutual support and trust.

Main duties of the job

The successful applicant will take the lead of a caseload of patients and be at the forefront of their care. You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the Inpatient Services and Home Treatment Team.

As a case manager you will assess health and social care needs and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.

You will proactively undertake and ensure rigorous risk assessment & risk management strategies are applied, helping to identify and mitigate risks as well as encourage individual growth and development for your patients through informed positive risk taking

You will apply specialist clinical knowledge and skills to carry out evidence-based interventions, which may be individual or in groups, which are underpinned by relevant theoretical approaches and in line with the Trust's Research and Development Strategy; to lead and supervise junior staff in this practice as required.

  • Current Professional Qualification in mental health nursing
  • Registration must be live with professional body
  • Evidence of post registration training and CPD

Desirable

  • Post qualification training in relevant mental health therapeutic skills
  • Training in Psychosocial Interventions or Family Therapy/Work training
  • Other relevant training e.g. substance use

Experience

Essential

  • Extensive post-registration experience
  • Experience of working in multidisciplinary mental health team with clients with acute/severe and enduring mental health problems

Desirable

  • Experience of working in a community adult mental health team
  • Work with people experiencing mental health crisis
  • Supervision of students and junior staff.
  • Audit and research

  • Excellent verbal and written communication skills, including report writing and presentations / IT skills.
