Are you a highly motivated and enthusiastic RMN interested in joining our Older Adult Home Treatment Service?
HTS work with service users presenting in acute mental health crisis, preventing hospital admission and supporting early discharge from hospital. The service works closely with both the inpatient ward and Community Mental Health Team, all based at the Woodland Centre, Hillingdon Hospital. The service runs 7 days a week 9-5pm 365 days a year.
Main duties of the job
We are a needs-led service and work with service users that are physically frail and have a mental health problem. People present with both primary mental health illnesses and dementia.
You will need to have post registration experience, preferably experience of working with older people with mental health problems and /experience of working in an NHS community-based team.
You will have excellent communication and interpersonal skills, with a sound understanding of current government legislation, the ability to work as part of a MDT and provide high quality care with evidence based therapeutic interventions.
Car owner/driver is preferable

The Woodland Centre is in the grounds of Hillingdon Hospital and has a 17 bedded Older Adult Inpatient unit, CMHT and Memory Service.

ClinicalTo triage referrals and allocate work to junior staff as appropriate.To provide a comprehensive mental health assessment for complex patients in the acute phase of a mental health crisis.To assess current risks and identify how the risk and patients mental health needs would best be met.
To formulate, implement and review individualised Recovery Care Plans with patients and their carers that aims to address their psychological, physical & social needs.To build and maintain therapeutic relationships with patients and carers and involve themin drawing up care plans.To work with patients care co-ordinator/ lead professional as appropriate to ensure continuity of care.To provide information and advice to carers to support them through the crisis period.To administer , supervise and monitor prescribed medication advising patients and carers on storage, administration and possible side effects.To have a sound understanding and knowledge of current legislation and itsapplication to service users with mental health problems e.g. Mental Health Act, Mental Capacity Act, Human Rights Act, Freedom of Information Act, Data Protection Act.To attend MDT meetings, Ward Rounds, CPAs and Mental Health ActAssessments
