Job summary

An exciting opportunity has arisen within Central and North West London NHS Foundation Trust for a highly motivated and enthusiastic Band 7 Team Leader to join our Community Mental Health Services.

Main duties of the job

This is a Band 7 Team Leader post which is open to the professions of Mental Health Nursing, Occupational therapy and Social Workers.

The role consists of leadership and supervision of the CMHT staff group, duty management, a small specialist clinical caseload, MDT meeting facilitation and support/ deputation to the CMHT Team Manager.

The post is for working 37.5 hours a week and the CMHT working hours are Monday to Friday 9am-5pm.

About us

The CMHT works with adults covering the Hillingdon borough and is based in Uxbridge. The base is close to major motorway networks and a short walk to the Metropolitan and Piccadilly Tube Lines.

The CMHT is a service user led service and works with adults that have mental health issues and require support with their physical health needs. Caseloads are a mixture of service users with severe and enduring mental health issues and acute mental health issues. The community services in Hillingdon are currently under transformation and therefore it is an exciting time to join the team when services are changing to be more responsive to the service users within the Borough.

The CMHT is a multidisciplinary team, comprising of mental health nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists and peer support workers. The team is supported by a good administrative system and offers an excellent work environment. The team has good links with local statutory and non-statutory services. The team works closely with our colleagues at The Riverside Centre at Hillingdon Hospital.

Job description

Job responsibilities

  • To provide day-to-day management support of the multi-disciplinary team in delivering community care to service users and their carers based on an intervention-based model of care
  • To work closely with the Team Manager and other senior workers to develop the Community Mental Health Team
  • To establish strong working relationship with the in-patient Service, Hillingdon HTT, Hillingdon Psychiatric Liaison Team and CNWL Single Point of Access.
  • To attend any required meeting to update on CMHT actions and feedback to CMHT any further actions required.
  • To monitor workload and ensure fair distribution according to the various team roles. To ensure adequate cover of patient care during absences.
  • To have oversight of the health-related functions with the Community Mental Health Team
  • To chair the morning zoning meetings as required
  • To facilitate HTT transfers in a timely way
  • To have oversight of zoning for patients and their plans
  • To attend monthly interface meetings and contribute to improving local care pathways.
  • To regularly undertake delegated managerial responsibilities and duties, and to deputise for the Team Manager in his/her absence or as required.
  • To competently access and interpret the CMHTS data dashboard.
  • To undertake complaints and serious incident investigations in accordance with the Trusts Policies.
  • To offer regular structured supervision to staff within the team, as well as providing informal support and advice to them and to any other team members in the course of their work.

To assist in the investigation of such matters, at the Service Managers request.
