Job summary

This is a rare opportunity to join our team of community nurses and support the ongoing development and delivery of learning disability services in Milton Keynes.

Milton Keynes Community Team for Adults with Learning Disabilities is an integrated NHS and Local Authority service. Band 6 Community Nurses are essential members of the team and work in collaboration with the wider MDT and Social Care Colleagues to optimise the health and well-being of people with learning disabilities through the application of evidence-based clinical practice and The Trusts' core values.

Main duties of the job

The post holder will be required to support the delivery of quality health care to adults with a learning disability in collaboration with their families, carers and primary care partners.

The post holder will manage their own caseload, working autonomously with a broad spectrum of need, providing skilled nursing support and effective planning, implementation and evaluation of interventions.

The post holder will be a Registered Nurse with a specific interest in the field of learning disabilities.

The post holder will have a key role in providing health facilitation and work to support primary health services in their provision of accessible services for people with learning disabilities.

The post holder will support student nurses and junior staff as required ensuring a rich learning experience.

The post holder will be experienced in the delivery of services that meet the needs of a diverse and multicultural community.

It is essential that you have a full driving license and access to a car as you will be working in the community.

Education and Qualification

Essential

  • Registered nurse Adult / Child / Mental Health / Learning Disability

  • Portfolio demonstrating clear evidence of professional development in relevant areas e.g. learning disabilities, Health Promotion, education and training, mental health/behaviours of concern

Previous Experience

Essential

  • Previous experience as a Band 5 or Band 6 nurse
  • Knowledge, experience and interest in the field of learning disabilities
