Job summary

Do you want to work in an enthusiastic team within a robust MDT delivering addiction treatment? CNWL is seeking an enthusiastic and committed clinical fellow in our Milton Keynes service: ARC-MK. The applicant will have completed their foundation programme (or internship / house officer post if outside the UK) and want to gain experience of Psychiatry (and especially Addictions Psychiatry) in preparation for entering or pursuing core training.

Interview date will be on Monday 6th February 2023

Main duties of the job

The post comprises a 40-hour working week. Clinical duties will be delegated and supervised by the Consultant Psychiatrist. Wednesday afternoons are allocated to Continuing Professional Development (CPD) activities, coinciding with the academic programme at our local mental health unit at the Campbell Centre, but also quality improvement, research and training. The post is based primarily at ARC-MK. The post is designed to be flexible and the post holder will be expected to respond to strategic developments and the needs of the service. The post holder will work closely with other doctors across CNWL in Milton Keynes. The post is non-resident.

About us

ARC-MK is a partnership model service led by CNWL, together with a voluntary sector service known as We Are With You. The service provides a comprehensive range of addiction treatments to Milton Keynes residents with drug and alcohol problems and their carers. The MDT comprises 1x consultant psychiatrist (part-time), 1x sector manager, 1x service manager, 3x team leaders (including a community engagement lead), 4.5x registered nurses (including 1.5x non-medical prescribers and 1x hospital liaison nurse), 11x recovery practitioners (including 1x hospital liaison practitioner), 2 x administrators and volunteers. Other than the consultant psychiatrist, there are no other medical staff within the team. The service is strengthening partnership working with mental health services, also provided by CNWL. Our holistic care model provides parity between mental health & physical health, working in a recovery focused way to support individuals through a comprehensive group programme complimented by prescribing pathways, including providing support to a Young Peoples' service. The service's overall caseload is approximately 600, with approximately 400 patients prescribed opiate substitute treatments.

Job description

Job responsibilities

To work as a member of the multidisciplinary team and participate in patient assessment and the delivery and evaluation of treatment. Responsibilities include assessment of new patients, review of existing patients and planning treatment with keyworkers.

To be responsible for day-to-day medical assessment and decision-making (with appropriate support and working within limits of competence).

To liaise closely with patients keyworkers, who will be primarily responsible for co-ordinating their care. The post holder may be required to hold a small caseload, although the post holder will primarily review existing patients collaboratively with their allocated keyworker. Follow-up reviews may be facilitated should the patients presentation change or require a care plan review.

The model of addictions services is different to community mental health outpatient clinics - referrals are from keyworkers or duty workers in the service and are jointly facilitated to support further assessment or management. The number of assessments per week varies depending on patient complexity, need or if patient is new or follow-up.

To provide clinical advice to team members on a 1:1 basis and within MDT meetings.

To manage community patients undergoing detoxification for drug and/or alcohol dependence.

To initiate, stabilise and maintain patients on opiate substitute treatment (OST).

To undertake mental health reviews on patients.

To provide health care appropriate to the patients physical needs and make appropriate referrals to medical colleagues.

To provide support to designated and prospective Non-Medical Prescribers.

To participate in providing training to staff at ARC-MK as well as any students (medical, nursing and social work) attending the service.
