BAND 6 COMMUNITY NEURO PHYSIOTHERAPIST - CAMDEN
We are delighted to advertise for a permanent post within our team, The Camden Neurology and Stroke Service. This post is an exciting opportunity to work within a dynamic setting across the borough of Camden and join a highly supportive and cohesive therapy team. The Service provides client-centred assessment/rehabilitation/disability management at St Pancras Hospital and throughout Camden community, including in clients' own homes, in therapy gym facilities or in wider community settings.

Main duties of the job
You will work in the following pathways:
- Stroke Early Supported Discharge,
- Long-term conditions,
- NeuroActive: exercise-based programme to prepare for transition to local gyms and classes
The successful candidate will provide evidence-based, client goal focused rehabilitation appropriate to the various specialist areas. We are looking for a highly motivated team worker with excellent communication skills.
You will need to have significant postgraduate experience, including substantial experience working in neurology and rehabilitation.
This post will allow the successful applicant to continue to improve specialist knowledge and clinical skills in a range of settings. The candidate will work alongside experienced multidisciplinary teams and be well supported by senior staff both informally and formally through supervision. There is a strong focus on personal and service development and you will take an active role in these.

The remit of the team is to provide rehabilitation and therapy intervention for adults with neurological conditions in the community. Patients will be over 18, Camden residents and unable to attend outpatient appointments or where treatment is more appropriate carried out in their own environment. The team promotes independence and community integration, and enables self-management where appropriate. Work with clients can vary from short episodes of care to longer-term support or rehabilitation, but always follows a goal orientated approach. The team is committed to a clinical governance programme and team members are encouraged to use an evidence based practice model and to participate in research and audit programmes. You will provide physiotherapy input in the Early Supported Discharge (stroke) Pathway, Long-term conditions Pathway and NeuroActive Pathway.
NeuroActive is a community exercise programme which creates an opportunity for people with neurological conditions unable to access mainstream leisure/exercise facilities to exercise.
