Job summary

Hillingdon Health & Care Partners is an integrated partnership which consists of, Hillingdon 4All (3rd sector partnership), Hillingdon Primary Care Confederation, CNWL NHS Foundation Trust, and Hillingdon Hospitals NHS Foundation Trust. The work is being progressed working via an alliance across all the providers alongside a Memorandum of Understanding with Hillingdon CCG

The Integrated Discharge Team is a collaborative and partnership approach to coordinating and planning the transfer of patients, including those with complex health and social care needs, back to their usual place of residence wherever possible following an episode in hospital.

The post holder will hold a significant number of operational responsibilities and will require; experience of working in the community; the development and maintenance of positive and productive relationships across current and changing team and organisational boundaries, often in pressurised situations; the delivery to timescale of key deliverables and performance targets; understanding of the context within which the key health and care organisations operate and how to effectively align their priorities, statutory requirements and local pressure.

Main duties of the job

Along with the Discharge Matron you will be jointly responsible for leading a robust, system-owned model of integrated discharge that ensures effective, person centered and integrated pathways of care out of hospital are embedded through interdisciplinary ways of working and clear, well managed processes and protocols.

To take a lead role in the strategic planning and service improvement of the integrated discharge service with support from health and social care management. The Complex Discharge team comprises specialist practitioners from across health and care organisations including the third sector.

To lead in the development of clinical standards, protocols, policies and procedures for discharge systems. To ensure up to date and evidenced based practice is performed by all members of the team by undertaking supervision, mentoring, and training and performance management of all staff.

To take a lead role supporting the health and social care system in maintaining capacity and flow across the whole system through the promotion of safe, timely discharges closer to home, to reduce the detrimental outcomes associated with a prolonged length of stay.

To ensure appropriate operational cover to deliver service and work operationally 50% of the time

To ensure best practice pertaining to patient discharge is embedded within team and promoted throughout the acute Trust and local system

To adhere to the acute Discharge Procedure, and associated policies relating to discharge planning.

To screening and oversight of referrals and improvement of information to health and social care community services

To ensure correct processes for identifying, calculating and reporting DTOC (Delayed Transfers of Care) in accordance with The Care Act (2014) on a daily basis are in place and monitored.

To ensure weekly DTOC is reported across acute, community and social care sites. This includes authority to mutually agree and sign off DTOC between the Trust, Local Authorities and relevant other organisations.

To lead the integrated team in the formulation and initiation of appropriate strategies, to minimise the risks associated with complex patient discharge.

To implement the Internal and External Escalation actions required when the Trust is experiencing difficulties with capacity and whole system flow.

To escalate patients and relatives/carers declining to leave a NHS bed and exercising the right to choice, to the Integrated Discharge Team in order for the Choice Policy to be enacted.

To provide leadership to the team for the implementation of quality initiatives to enhance patient experience & satisfaction, such as R2G (Red to Green), Stranded patient, 10 before 10 and PJ Paralysis.

To liaise and sustain partnership working with internal departments across acute, community sites and community services.

To promote integrated and collaborative working with all relevant, external stakeholders and partners including the 3rd sector.

To liaise and communicate with patients/relatives/carers where discharge is outside the remit of team colleagues and usual service criterias.

Establish trusted assessor agreements with external partners.

To sensitively challenge conventional thinking and cultural ways of working that contributes to delays or barriers to the discharge process.

To lead on complex cases requiring detailed knowledge of health, social care policy and legislation; escalating where necessary to senior management if required

Foster relationships with out of county organisations and services, promoting cross boundary working.

To ensure the observance of high standards of discharge planning within available resources, setting, monitoring and evaluating standards of discharge.
