Job summary

The CNWL Hillingdon Podiatry team are exploring new ways of meeting the demands of the service and are keen to introduce the role of a Podiatry Specialist Nurse. The ideal candidate will either possess or be willing to undertake training to extend their scope of practice to support the 'Diabetic and At-Risk Foot' for wound care and debridement. This role is one of a few new roles created as part of the Podiatry service redesign to include other health practitioners. This post involves working with a team of Specialist Podiatrists to ensure neurological and vascular assessments are completed, sharp debridement, appropriate wound care involving VAC therapy and Larvae therapy is supported in the community settings to deliver the best care to our service users. This successful post holder will also be line managing other nurses and Podiatry assistants in the team and carrying out Management and Clinical Supervision on a regular basis. Nurses with sharp debridement skills are particularly welcomed and encouraged to apply to support escalation and review with the Specialist Podiatrists and the local multidisciplinary foot team. However, nurses with enthusiasm, appropriate experience of working within tissue viability or have a good knowledge of Diabetes and are willing to undertake training are also encouraged to apply.

Main duties of the job

The post holder will assist the Podiatry team in managing a clinical caseload, in the community by supporting patients with Podiatric needs and assisting in managing the tissue viability of the foot, to ensure delivery of cost and clinically effective Podiatry service to patients, carers and health care professionals.

The caseload will consist of high risk house bound patients and ambulant patients seen within the three Podiatry High Risk clinics based at Eastcote, Hesa and Uxbridge Health centre.

The post holder will be required to work across a variety of settings within the borough of Hillingdon, including clinics, Care Homes and in patients own homes. The role also requires adaptability to support the changes within the department.

About us

The post holder will be working alongside a team of Specialist Podiatrists, Community Podiatrists and Podiatry Assistants in protecting the "At risk Foot".

This role involves working closely with other specialist nurses, tissue viability team, district nurses and the community ambulatory team. This is a full time post but other working patterns will be considered that meet the needs of the service for the successful candidate

  • Act as a role model in expert practice facilitating and supporting the delivery of effective care by use of clinical judgement and decision making skills.
  • Provide supervision and line manager support to a Band 5 Podiatry Nurse
  • Provides support and management to the Podiatry team, ensuring effective clinical standards, and development of individual team members.
  • Have responsibility for the assessment of care needs and the development, implementation and evaluation of care programmes both simple and complex for and with individuals and their families.
  • Identify individual and collective tissue viability needs of the foot through observation, assessment and discussion such as Doppler studies, advanced Doppler studies and advanced wound assessment applications.
  • Analyse and interpret collected information and collate as part of holistic assessment to identify patients needs, aetiology of wound, prognosis and then facilitate management programmes to address patient need.
  • Assist the Podiatry team in managing complicated aspects in relation to tissue viability of the foot to include debridement, larvae therapy, vac therapy and redressing
  • Prescribes and Implements evidence based practice at all times, including VAC, bio-surgical debridement, and intermittent pneumatic compression.
  • Liaise with other organisations, Voluntary, Statutory and Health to provide holistic care.
  • Assist in care plan setting for Podiatry patients.
  • Engages patients and their carers in all aspects of care planning and delivery
  • Implements the framework of informed consent
  • Provides health advice and support for other agencies.
  • Work with patients, carers and families to enable them to develop self-care skills with respect to health and health services
  • Document consultations and assessments and practice to incorporate both audit and the setting and monitoring and evaluation of standards of care.

  • Implements the National Service Frameworks and NICE guidelines
