Job summary
This busy, essential and innovative service is looking for an experienced mental health nurse to take up an Admiral Nursing post within Kensington & Chelsea and Westminster Memory Service. If you can add to the energy, development and delivery of Admiral Nursing in a modern memory service we would like to hear from you. You will be appropriately qualified and have significant experience of providing assessment, nursing care and interventions for people who are living with dementia and their Carers. Admiral Nurses work with carers of people with dementia providing information, emotional support and practical advice. They also provide a Consultancy Service to staff working within this complex field of dementia care.This a community-based post so you will need the ability and confidence to act autonomously in the community when seeing carers in their normal place of residence. You will be a good team player, able to build therapeutic relationships with service users in addition to being able to develop good working relationships with other health and social care professionals.Kensington, Chelsea and Westminster Memory Service is a community based multidisciplinary team delivering a memory assessment and treatment service. Based in central London it brings diversity of population and the challenges of supporting older people in inner city environments. The team is well established and encouraged all members to have full involvement with service development.
Main duties of the job
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage & support vaccination as this remains the best way to protect yourself, your family, your colleagues & of course patients & service users when working on our healthcare settings.
- To provide specialist nursing assessment, evidenced based intervention, practical advice & emotional support for family carers of people with dementia.
- To carry out joint assessment of the person with dementia where complex needs within the family are identified (e.g. difficulties with adjusting to diagnosis and/or atypical diagnoses; significant changes in the mental and/or physical health of the person with dementia; family breakdown; carer stress, anxiety and depression; managing risk).
- Offer expertise and guidance to professional colleagues, the general public & others in supporting family and relationship-centered approaches and best practice in dementia care.
- Assist with the development, evaluation and audit of this specialist area of practice and of the Admiral Nursing service.
- Provide up-to-date clinical advice, support, liaison, and in negotiated circumstances education & training to other staff/agencies.
- Use the Admiral Nurse competency framework to support the development of own Admiral Nursing practice.
- Participate in & make effective use of Dementia UK Clinical Supervision and Practice Development.
- Provide clinical supervision to others in negotiated circumstances
