CAMHS Urgent Care Band 5 Specialist Mental Health Practitioner
Are you an ambitious and motivated newly qualified mental health nurse who is seeking to take your career to the next level? In CNWL Camhs Urgent Care team, we have an exciting opportunity for an exceptional candidate, with demonstrable potential to join the CAMHS Urgent Care team in London to cover Chelsea and Westminster A&E/St Marys hospital..
You will work under the supervision of a mentor to support you in broadening your skills, responsibilities and competencies. Your mentor will also help you to identify areas where you may need to develop, and provide you with the opportunities to attend further training.
You will receive weekly and monthly management and clinical supervision sessions for feedback, support and guidance to ensure you are adhering to the best clinical practice, policies, and protocols.
You will be working in an experienced Camhs multidisciplinary team to support you in your development. The candidate post will provide you with the tools, training, clinical supervision, and support to excel in your professional journey.

Main duties of the job
Key Responsibilities:
Attend A&E to do generic camhs assessment for children in crisis.
Clinical risk assessment, and management plan.
Formulate safety/discharge plan.
Offer 7 days follow up appointment reviews.
organise and plan professional meeting.
Work within a multidisciplinary team.
Collaborate with experienced nurses, and healthcare professionals to enhance your clinical skills and knowledge.
Attend regular clinical supervision with a dedicated supervisor.
Participate in specialised training programs and workshops to expand your expertise in crisis care, and other nursing specialities.
Engage in evidence-based practice to contribute to the advancement of nursing care in UCT.
Take on leadership responsibilities and assist in mentoring other student nurses in the team.
Continuously assess and improve patient care delivery to ensure the highest standards of care.

Job responsibilities
To provide comprehensive, culturally appropriate, evidence-based assessments to establish eligibility for health and social care services and consult/refer to other services when appropriate.
To key work an allocated caseload of service users with complex mental health, incorporating the following:
To provide all service users with a specialist mental health assessment encompassing health and social care needs in line with national and local policy; the Care Act 2014 and Section 117; and any other relevant.
To undertake continuous Risk Assessment and Risk Management and Safety Plan for children and young people presenting in crisis.
Respond to A&E for crisis assessment within agreed period of time to meet KPI target.
To formulate, monitor and review service user care plans.
To provide direct therapeutic care to children and young people presenting in crisis to A&E or other community settings, using evidence-based nursing skills and psychosocial interventions.
To co-ordinate and manage all aspects of care, including overseeing work carried out by statutory and non-statutory agencies. To offer supervision, training, and guidance to external agencies and internal junior staff members as required.
To identify cultural, spiritual and communication needs of users within a diverse community and ensure that these needs are addressed in the care plan.
To co-ordinate a multidisciplinary response to crisis situations.
To ensure regular reviews of the care plan and ongoing monitoring of all aspects of treatment and services provision.
To provide evidence-based medication management in line with NMC guidelines and Trust Policy and to develop user centred medication concordance.
To assess, plan, implement and evaluate the care provided to users, in collaboration with users,
carers, GPs and other members of the multidisciplinary team.
To identify carers and to offer statutory Carers Assessments and Care Plans. To review Carers Care Plans in line with policy and statutory requirements.
To work according to Children Act 1989..
To work in partnership with other agencies i.e. schools, social services, community camhs teams, Acute hospital, and other agencies.
To identify Safeguarding and Child Protection issues and to implement procedures as required.
To prepare and present clinical reports for service users when required.
