Job summary

An opportunity has opened up for aspiring RMN's to join our Older People's Community Mental Health Service at the heart of Kensington and Chelsea, where you will be afforded the time and space to develop into a more enhanced and effective community practitioner.

We have an opportunity in our CMHT/HTT. Both teams are integrated within each other which allows for a more fluid way of working and also allows for you to work across both services on the Bank should you wish.

  • HTT service- 7 days a week with working hours of Monday to Friday 8.30am-7pm and Saturday and Sunday 9am-5pm, no night shift
  • CMHT service- Monday to Friday 9am-5pm with the opportunity to do bank shifts with HTT.

Community Services are becoming fast-paced and rapidly changing to support the 'Out of Hospital Care' strategies. You will therefore need to demonstrate resilience and the ability to adapt to the changing landscape and opportunities.

Based in Kensington and Chelsea, we are near Portobello market with its vibrant, beautiful shops, Notting Hill Gate and Holland Park where staff can take advantage of the more peaceful areas.

Main duties of the job

CMHT - You will be expected to case manage an allocation of patients, monitor patients' mental state, promote independence, promote social inclusion, aid the recovery process, and reduce hospital admissions.

HTT - This role includes gatekeeping, home treatment providing alternatives to inpatient hospital care, and monitoring patients' safety and well-being to promote independence in the community, to reduce relapse rates and re-admissions. Facilitate early discharge and discharge follow-up.

You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services, and the HTT backup across the service.

About us

  • CMHT service is Monday to Friday 9am till 5pm; opportunity to do bank shifts with HTT as the teams work very closely together.
  • Both hubs are led by the same Consultant which allows for a clear understanding of each individual patient and allows for the ability to work collaboratively together to provide the best possible outcome for the patient.
