Job summary
We have created this new position for a Senior Sister / Charge Nurse within the Virtual Ward to bring even more experience and knowledge into the team and provide a day to day oversight into the Virtual Ward. You would be the first ever Camden Virtual Ward Senior Sister / Charge Nurse, meaning you can be part of shaping what the role is and how it functions.
Virtual wards allow patients to get the care they need at home safely and conveniently, rather than being in hospital. Camden's Virtual Ward has two 'prongs', medically led and nursing led.
This post will be working within the medically led pathway. We see a variety of patients where they have been discharged early under the care of VW, having otherwise faced an extended admission in the acute hospital bed. Alternatively, patients who were under the care of the UCR pathway may have been escalated to the VW for such instances as Delirium or other conditions requiring medical input.

Main duties of the job

Day to day oversight, running the nursing aspects of the Virtual Ward.

Undertaking review of patients on remote monitoring technology.

Ensuring rostering and allocations are up to date (at all times - DocAbode).

When required, escalating staffing needs to service's Lead and/or Deputy Lead Nurse(s).

Managing and overseeing patient/relative/carer phone enquiries .

Contributing to Information and Clinical governance (audits, Datix reviewing, managing feedback).

Ensuring patient safety, being a point of clinical escalation for the Nursing team members out seeing patients.

Be prepared to undertake clinical visits when required, ensuring safety of oversight is not jeopardised.

Ability to undertake remote triage / clinical review of patients using mediums such as telephones and video calling.
