Great Chapel Street Medical Centre Band 7 Nurse
We currently have an exciting opportunity for a Band 7 Nurse to join the Great Chapel Street Medical Centre. The Medical Centre is situated on the 4th floor of the Soho Centre, 1 Firth Street W1D 3HZ.
We are looking for a compassionate, highly motivated, enthusiastic and clinically confident Practitioner with excellent communication and interpersonal skills. As a successful candidate you will you will be expected to engage effectively with service users and their carers to feel safe and cared for.
You will need to demonstrate strong vibrant leadership skills to co-ordinate the work alongside the team at Great Chapel Street. You will work clinically with the Team providing clinical input and leadership to the MDT at the practice.
This is an excellent opportunity for the right candidate to influence and deliver high standards of care, be creative and dynamic in the development of the Service.

Main duties of the job
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.
You will manage a small case load of patients and be responsible for ensuring that all KPI are met. You will form part of the Primary Care MDT team assisting case reviews, triage of those presenting with mental health needs, and supporting your GP colleagues with referring clients to Mental Health Services as appropriate. For those with SMI and where applicable to the skilled professional, the post holder will support antipsychotic medication management.

- Experience and knowledge of Mental Health Issues.
- Knowledge of a range of acute and long term conditions
- Excellent Communication skills
Desirable
- Educated to degree/diploma level
- ACP Training or working on it
Previous Experience

- Substantial years post qualification experience in mental health
Desirable
- Other relevant post registration experience.
- Audit and Research experience
- Experience of supervising staff
- Knowledge of cognitive behavioural skills, brief solution focused therapy

- Ability to communicate clearly and effectively (written and oral)
- Provide specialist bio-psycho-social and mental state assessment for patients either as part of the transfer of care process or following request by primary care team.

- An understanding of performance management systems and the importance of achieving targets
