Job summary

Band 5 Community Mental Health nurse- Development post - Hillingdon

This is an exciting opportunity to work in a CMHT as either a newly qualified mental health nurse or an existing band 5. The post will offer an additional support programme to develop, suitable candidates to become safe and effective community mental health nurse.

Main duties of the job

We are service user led services and work with adults that have mental health issues and require support with their physical health needs. Caseloads are a mixture of service users with long and enduring mental health issues and acute mental health issues.

The CMHTs are multidisciplinary teams, comprising of nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists and peer support workers. The teams are supported by good administrative systems and offers excellent working environments. The teams have good links with local statutory and non-statutory services. All the CMHTs work closely with our colleagues at The Riverside Centre at Hillingdon Hospital.

The role which consists of management of a CPA caseload, medication administration/ monitoring, Duty work and attendance to MDT meetings.

About us

The aim is to support the band 5 nurse to develop the skills to be ready to apply for a band 6 nurse role in 12-18 months.

The support programme will provide the following:

  • Work based support & supervision.
  • Additional coaching and supervision from CNWL Education team.
  • Preceptorship programme 10 sessions (If within first year of qualification)
  • Addition monthly sessions to support role development.
  • Programme content & skills: Coaching, risk assessment/management, assessment, safety planning, suicide prevention, relapse prevention, report writing, working with families/carers and psychological interventions.

To ensure the mental health needs of adults with mental illness referred to Hillingdon CMHTs are appropriately assessed, supported and a high level of quality care is delivered within available resources.

The Community Mental Health Nurse /Care Co-ordinator will work as a member of the CMHTs in Hillingdon.

The post holder will assess and support a defined caseload of service users living in the community who have a range of complex mental health needs.

They will demonstrate the ability to travel to see service users in the community/ as inpatients and within their home environments and under lone worker policy and guidance.

The post-holder will also be part of the CMHT duty rota and will also carry the function of duty worker on a rota basis.

They will undertake holistic community assessments of service users referred to the team utilising the Initial Assessment process, Risk Assessment tools, CPA, Mental State examination and Integrated clinical notes system (SystmOne).

The post holder will promote an increased awareness of mental health issues for adults with mental illness within the community and work closely with primary care and statutory agencies.
