Job summary
Horton Rehabilitation services, Central and North West London NHS Foundation Trust are seeking two dynamic and enthusiastic Specialty Doctors to join our specialist inpatient rehabilitation services (1 WTE and 0.5 WTE). The successful candidates will be passionate about delivering high quality care to people suffering from severe and enduring mental health difficulties.
Main duties of the job
The post holder will be expected to provide in-patient care for the patients admitted to the Horton Rehabilitation Services under the supervision of the Consultant Psychiatrist. The post is for Westfield House (Managed Access Unit)
In addition, the post holder will have the opportunity to undertake assessments, in conjunction with nursing or other members of the multi-disciplinary team, of those referred to the service. This will include providing support and advice to referring services should admission not be required. This post is part of a fully integrated multi- disciplinary team as well as support and administrative services.

The Horton Rehabilitation Service is based in Epsom, Surrey and is well connected to London via the Victoria or Waterloo railway lines. Horton Rehabilitation Service provides a stepped care model of inpatient rehabilitation care for CNWL NHS Foundation trust and comprises of the following typology:
- Ascot Villa- 15 beds; mixed sex - High Dependency Unit Rehabilitation
- Birch Villa- 15 beds; male - High Dependency Unit Rehabilitation
- Westfield House- 25 beds - mixed sex; High Dependency Unit Rehabilitation
- Community Inpatient Rehabilitation model - Open 16 beds mixed sex;

Job responsibilities
- Detailed clerking, history taking and initiation of all necessary investigations and assessments for inpatients.
- The assessment and initial treatment of new admissions, performing physical examinations and the organisation of clinically indicated physical investigations.
- The ongoing assessment and management of inpatients, including contributing to multidisciplinary reviews by presenting updated clinical information on patients and documenting the multidisciplinary discussion.
- Establishing, initiating and evaluating treatment plans and risk assessments in collaboration with the multi-disciplinary team and under the supervision of the Consultant Psychiatrist.
- Attending multidisciplinary team meetings, CPA meetings, and Professionals meetings as an active member of MDT
- Interviewing relatives and informal carers of patients as required.
- Undertaking the relevant referrals to other disciplines and services both within the multi-disciplinary team as well as to services and resources external to the service
- Observation of Trust, and local policies and procedures; including those related to medicines management and physical healthcare.
- Preparation and presentation of medico-legal reports, including those for Managers Hearings and Mental Health Review Tribunals for in-patients.
- Involve in Mental Heath Act assessments and make recommendations for detention as appropriate
- The post holder will be expected to assess in conjunction with nursing, or other relevant personnel, those referred to the service.
- The post holder will complete full risk assessments of all in-patients in conjunction with the MDT.
- To participate in the patient administration information system of the Service (System1) with the multi-disciplinary team
- To take accountability for physical health monitoring of the patients and to liaise as and when required with the visiting GP and General Hospital / specialist secondary services

- Previous Experience in a mental health rehabilitation or Secure Setting
