The summary of the Carelink Nurse post is as follows:
To be accountable for the line management, supervision, support and development of Community Health Support Workers (CHSW) to ensure they are working to the standards which the people using our services deserve
To support and be the lead on the development of CHSW nursing and medication competencies, to case manage all palliative care patients that are referred to the team and where appropriate liaise with Continuing Care and Palliative Care Teams To be responsible for providing support, mentoring and leadership to apprentices and student nurses
To lead on clinical and service audits and other service development activities in support of the service manager
To ensure service operational policies, procedures and clinical protocols are in place and effectively adhered to by the team and to undertake highly specialised assessments and treatment interventions in the community to adults over the age of 18
To be an active member of the larger multidisciplinary Integrated Adult Service
The NMC plays an important role in safeguarding the public by regulating the work of nurses and all nurses, including Band 7 nurses, are expected to register with the NMC each year and adhere to the NMC Code of Conduct.
Main duties of the job
To work as an autonomous lone practitioner, case managing clients in the communityTo independently screen and assess phlebotomy, reablement and palliative care referrals from a variety of sources ensuring that risk assessment, planning and implementation of care meets professional and clinical standards and guidelines
To carry out timely assessments and rehabilitation of community clients to prevent hospital admission and re-admission and ensuring smooth facilitation of early discharge home from hospital
To lead within Carelink on maintaining the local administration of medication policy,
To monitor the delivery of planned care to clients in their own homes ensuring that the CHSW have the required skills to provide phlebotomy, enabling and 'end of life' care to clients, and to meet the complex needs of these clients
To facilitate daily handover of client progress, always ensuring that client goals are at the forefront of the service provision.
Lead on clinical audits, service quality assurance and provide support with clinical governance review.
To support the Team Leaders to meet daily service staffing requirements in establishing service priorities and liaising with provider care agencies.
To educate clients, carers and family members on the clients care planning and intervention
About us
Carelink is a community based care provider within CNWL, The team is staffed with Community Health Support Workers (CHSW) who provide short term care for adults over 18, although most of the clients are older adults. In conjunction with the service manager and the deputy manager the post holder will be responsible for overseeing the delivery of individualised care for admission avoidance and supported discharge clients and for those clients that are under the 'end of life care' pathway.
Carelink is a 24 hour care provider, which operates a three shift system. The early shift is from 7am to 3pm and the late shift is from 2pm to 10pm, a daily handover is facilitated between 2.30pm to 3pm. The service also, provide a night sit in service from the hours of 10pm to 7am, making it a truly 24 hour service.
The post holder will predominately be working Monday to Friday 9 am to 5pm, however, they will be required to work the occasional late and weekend shift.
