Job summary

Are you a highly motivated and enthusiastic RMN interested in joining our Community Mental Health team working with older people in Hillingdon? We are a needs-led service and work with service users that are physically frail and have a mental health problem. Caseloads are a mixture of service users with long and enduring mental health issues, acute mental health issues or issues relating to memory loss. You will act as a Care Co-ordinator or Lead Professional dependent on the complexity of the case.

You will need to have post registration experience, preferably experience of working with older people with mental health problems and /experience of working in an NHS community-based team.

You will have excellent communication and interpersonal skills, with a sound understanding of current government legislation, the able to work as part of a MDT and provide high quality care with evidence based therapeutic interventions.

Car owner/driver is essential

Main duties of the job

1. Assessments and treatment of patients with patients with Primary Mental Health conditions and/or Dementia living in the community.

2. Assessment & management of risk.

3. Medication management

4. Supporting carers & family .

5. Liaising with statutory & non statutory agencies.

6. Acting as a Duty Worker & triaging new referrals .

7. Promoting Older Adult Mental Health services in Hillingdon.

Job description

Job responsibilities

1.To manage and have Care-coordinator/Case Manager responsibility for a defined clinical caseload of service users with complex mental health problems living in the community. This will include service users subject to the Care Programme Approach and under Lead Professional care.

2.To undertake holistic assessments of service users utilizing the integrated notes system, , Risk Assessment , Mental State Examinations and Physical Health checks.

3.Supervising and administering medication as necessary, observing effects and side effects, regularly updating knowledge on medication, also, ensure the policy for carriage and administration of drugs is strictly adhered to.

4.To work collaboratively with carers in the care & treatment of the service user and offer carers assessments as required.

5.Ensure that all views of service users and carers are effectively sought, channelled and acted upon, including the efficient actioning of compliments and complaints in accordance with the Trust policy.

6.To act in a manner to raise standards of care and make the services culturally sensitive, relevant and accessible to meet the needs of a diverse population.

7.To be conversant with the Safeguarding Adults procedure and alert to signs of possible abuse.

8.Help service users to understand the ageing process and how it affects their health and lifestyle. Assist Service Users in adjusting their lifestyle to maximize their comfort, dignity and independence.

9.To promote the work of the CMHT and an understanding of mental health issues affecting older people within the catchment area.

10.To participate in the Duty service for the CMHT and take responsibility for triaging referrals when on duty.

11.To maintain up-to-date clinical records electronically in line with agreed local policies and procedures, NMC guidelines and CNWL policy.

12.To ensure that own professional practice and clinical knowledge is maintained at all times in accordance with NMC guidelines and CNWL policies and procedures.

14.To acquaint the Team Manager with professional concerns and promote professional practice at all times.

15.To work in partnership with the primary care team, statutory and voluntary agencies to ensure that the appropriate care needs of older people with mental health problems are met.

17.To participate in CNWL developments and transformation of older adult services as required
