Job summary

We are seeking a Band 5 Nurse Practitioner who has an interest in Substance Misuse and would like to gain furtherexperience working in substance misuse to join our dynamic team. We work with partners and professionals from a multi-disciplinary approach to provide a great service and have many specialist sub teams such as Rough Sleepers, Young People, and Criminal Justice. An amazing opportunity to learn about all fields within addictions and to work collaboratively with various teams.

The Addiction Recovery and Community Hillingdon (ARCH) offer a range of treatment for adults and young people who are residents or have a GP in the borough of Hillingdon.

ARCH provides assessment and treatment for both substance misuse problems and any associated mental health issues. This may include substances such as heroin, club drugs, amphetamines other stimulants or alcohol addiction.

As a a nursing team we offer a range of services including specialist health and wellbeing clinics, expert advise and support to the team and a holistic learning environment.

Main duties of the job

Job Purpose

The post holder will undertake assessment, care planning and the implementation and evaluation of specialist treatment to clients with drug and/or alcohol problems. He/she will perform duty assessments to clients that are new to the service, in addition to acting as keyworker to a defined allocated caseload of clients with a low to medium degree of complexity, under the supervision of a designated Band 6 Practitioner within the team.

The post holder will be responsible for ensuring that the delivery of care and management of this client group is done in compliance with HCC Quality Standards.

Job description

Job responsibilities

  • To work as a member of the multidisciplinary team that provides a range of specialist services to problem drinkers and problematic drug users attending the service/unit, including harm minimisation approaches, motivational interviewing, relapse prevention, health education, detoxification programmes, pre and post test discussions (re BBV), key working and group work.
  • To make continuous assessments, under supervision, of the psychological, emotional, social and rehabilitative needs of clients, taking into account the needs of family, local community and developing systematic plans for care and evaluation of treatment
  • To provide case management and direct clinical care to a defined number of clients as agreed with the Team Co-ordinator and under the supervision of a designated professional within the team. This includes assessment, planning and providing specialist care. It is expected that the caseload will consist of clients of low to medium complexity and the postholder will take a predominant role in providing duty assessments to clients who are new to the service
  • To be able to assess and case manage drug-using clients, applying the precepts of harm minimisation,
  • To use agreed and established Outcome Monitoring Tools and any other relevant quality measures.
