In our quest to deliver patient-focused quality care we are looking to recruit a highly motivated, innovative and enthusiastic Primary Care Service Lead to join our new Integrated Healthcare Service team at HMYOI (Young Offender Institution) Cookham Wood in Rochester. Main duties of the job

The post holder will be an experienced healthcare professional who also has excellent staff management skills. They will support the Head of Healthcare to manage and develop the Primary Care team to achieve a targeted and age/developmentally-appropriate service, delivered in accordance with evidence-based practice and local policies and procedures. This includes working closely with the wider integrated healthcare team managers to support the delivery and implementation of the trauma informed framework for integrated care (SECURE STAIRS) within the establishment.
The post holder will proactively manage and lead the Primary Care team to further develop services such as long-term condition management; nurse led minor ailment clinics, sexual health clinics and immunisation programmes. In addition, the post holder will have a key role in designing and implementing the prison wide health promotion strategy and 'Positive Health Programme'.
