Job summary

The post holder will work as a member of a Brent Older People and Healthy Aging Service, delivering community mental health assessment and intervention. The post will have a main focus on initial assessment, intervention, review and care coordination dependent on the needs of their caseload.

Community Mental Health Team focus - The post holder will be responsible for the assessment and management of a defined caseload of service users living in the community who are experiencing functional mental illness. A small percentage of their caseload may present with additional challenges of cognitive impairment or have a range of complex mental health needs, including dementia.

The post holder will undertake holistic community assessments of service users referred to the locality teams utilising the Assessment, Risk Assessment tools, Mental State examination, care programme approach and lead professional care frameworks.

The post holder will offer a variety of therapeutic interventions including supportive therapy, medicine management and problem solving techniques. They may participate in the duty system for the team along with other members of the MDT and attend all relevant clinical reviews.

Main duties of the job

  1. To manage and have Care-coordinator / Lead professional responsibility for a defined clinical caseload of service users with complex mental health problems (functional and organic) living in the community. This will include service users subject to the Care Programme Approach and under Lead Professional care.
  2. To undertake holistic assessments of service users utilizing the integrated notes system, which will incorporate the Single Assessment Process, Risk Assessment and Mental State examinations.
  3. Supervising and administering medication as necessary, ensuring the policy for carriage and administration of drugs is strictly adhered to. (RMN/RGN's only)
  4. Monitoring compliance and observing effects and side effects of medication, regularly updating knowledge on medication.
  5. Ensure that all views of Service Users are effectively sought, channelled and acted upon, including the efficient actioning of complaints in accordance with the Trust policy.

CLINICAL RESPONSIBILITIES:

  2. To clinically manage a defined caseload of service users with complex mental health problems in a community setting including service users subject to the Care Programme Approach.
  3. To deliver appropriate therapeutic interventions to service users utilising a range of evidence-based therapies e.g. bereavement counselling, medicine management, anxiety management, relaxation techniques and problem solving techniques.
  4. To respond to urgent and unplanned clinical events involving service users known to the CMHT and on own clinical caseload, utilising analytical and problem solving skills to ensure the situation is managed safely and effectively.
  5. To have an in-depth understanding of mental health pathology to manage service users with complex mental health problems.
  6. To formulate individualised Care Plans with service users and their carers that aims to address their psychological, physical, spiritual & social needs.
  7. To act as Care Co-ordinator to service users on the Care Programme Approach to ensure the appropriate delivery of care is received by this vulnerable client group, whom have complex needs.
  8. To monitor service users and advise both them and their carers on anti-dementia and psychotropic medications, observing efficacy and monitoring any adverse reactions. To report and act on any adverse reaction observed in conjunction with the General Practitioner or Consultant Psychiatrist, as appropriate.
  9. To administer depot medication to service users and monitor any adverse reactions or side effects or changes in mental state.
  10. To regularly advise other professionals on medication management issues e.g. side effects, recommended medication dosages, changing medication. (In consultation with the Consultant Psychiatrist or General Practitioner, as appropriate).
  11. To regularly attend and participate in clinical reviews.
  12. To utilise clinical supervision and reflective practice in the management of own clinical caseload.
  13. To participate and contribute to regular reviews of own performance through Appraisal and Development Review Process in conjunction with your supervisor and manager. To agree and implement a service and personal development plan.
