Home 1st Therapy are looking for enthusiastic Specialist Physiotherapists, who want to put the fun and innovation back in to rehabilitation!
Due to an increase in funding, for further development of the Urgent Care Response (UCR) and Virtual Wards pathways, we need to expand our team.
Working in close partnership with our stakeholder colleagues, Home 1st Therapy provides holistic physical health and therapy assessment and treatment to adults living in Milton Keynes.
Our service spans rehabilitation pathways in both community and inpatient settings. Admission avoidance, 'Getting People Home' supported discharge, Inpatient Rehab, Pulmonary Rehab, Staying Steady falls prevention and Community based therapy are our key areas of work and provide an excellent opportunity to broaden your skills, knowledge and clinical decision making.
An awareness or experience in Frailty, UCR and discharge support would be of benefit, but as Home 1st proudly support the development opportunities for individual staff members, practical experiential learning is ongoing .
The team are keen to continue their support for personal development in rehabilitation based Apprenticeships and academic progression to ACP roles. We actively encourage participation of our therapists in peer support meetings, supervision and appraisal objectives, completion of QI projects and in clinical educator mentorship for AHP students on placement.
Main duties of the job
- To provide physiotherapy assessments and deliver person centred rehabilitation and individualised goals.
- To undertake all aspects of clinical duties as an autonomous practitioner.
- Working in rotations within the Community, assisting in admission avoidance assessments, the Getting People Home pathway to facilitate early supported discharge from the acute hospital and Inpatient rehabilitation pathways.
- To communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within Home 1st, as well as outside agencies.
- Ability to work in capacity as a Trusted Assessor.
- Participate in development of integrated pathways

- To be professional and legally accountable for all aspects of your work including the management of patients in your care.
- To use specialist professional and clinical knowledge across a range of procedures based on a sound knowledge of evidence-based practice and treatment options, using clinical assessment, reasoning skills and knowledge of treatment skills.
