Job summary

Community Mental Health Services in Milton Keynes have undergone transformation and are pleased to announce the Milton Keynes Community Mental Health Hub.

The Adult Mental Health Community redesign is part of an initiative to improve the quality of care received by people with mental health needs in their local community. The national direction of travel, outlined in the NHS Long Term Plan, is towards the provision of more integrated, personalised and place-based care.

An exciting opportunity has arisen for a Band 7 CAMHS Transitions Senior Practitioner to work across the Community Mental Health Hub ensuring that young people under the care of CAMHS have a transition period of up to 12 months which should start form aged 17 and 6 months.

Main duties of the job

The post holder will need to have an excellent knowledge of Mental Health and current mental health legislation.

The postholder will need to demonstrate the ability to support and apply a holistic approach to assessing and supporting the health and social needs of people experiencing severe and enduring mental illness.

To provide emotional and practical help, advice to young people and families

To screen referrals made to the Single Point of Access team for clients under CAMHS, to sign post clients not deemed suitable for The Hub. Clients who are suitable for The Hub, to lead the transition process.

To act as keyworker for clients transitioning from CAMHS to adult services

To work with the CAMHS transition Family Support Worker in supporting CAMHS clients and their families during a transitional period from CAMHS to adult services.

To work closely with families to develop skills and techniques to support in the transition process.

To work with Placements Team and CAMHS for those clients in hospital.

Education and Qualifications

Essential

  • Core Profession (Mental Health Nurse, OT, Social Worker, Clinical or Counselling Psychologist) and current live registration
  • Further training relevant to the care of clients with Personality Disorder
  • Mentor/Supervisor qualification i.e. 998 or equivalent

Desirable

  • Degree or Diploma relevant to the field of Personality Disorder
  • CBT / DBT for Personality Disorder

Previous experience

Essential

  • Extensive post qualifying experience in the NHS.
  • Experience of working therapeutically with people with Personality Disorder or challenging behaviour
  • Experience of using CBT/DBT/MBT interventions with people with Personality disorder

Aptitudes, attitudes and personal characteristics

Essential

  • To show an advance understanding of risk management practice involved with clients who have Personality Disorder.
