To assess, plan and review health and social care needs service users with an offending history. The post holder will work as a Care Co-ordinator/ care manager as part of a multi-disciplinary team.

Main duties of the job
If you are a CMHN/ Social Worker/ OT with a strong interest and /or experience in working with Forensic Mental Health then KCW REFoCuS could be the Team for you to join.
We are a Forensic Community Service based in Ladbroke Grove and our service user group is spread throughout the Kensington, Chelsea and Westminster boroughs.
We are a MDT who provide a complete Health and Social Care service to our service users working to a recovery based model. We have access to the full range of local resources and accommodation options for service users. We plan discharge from hospital settings, offer joint working and a risk assessment /management service to local community and inpatient teams. We supervise service users in the community on various sections under the Mental Health Act, and work with the Ministry of Justice and Probation services in regards to Criminal Justice restrictions. We work to maintain wellness and to step service users down to less intensive mental health services and on to full independence.
You would be supported to undertake specialist training as necessary, and be offered regular supervision and reflective practice in order to support you in this role.

Job responsibilities
- To ensure that professional practice is in accordance with the relevant Registration body and relevant Mental Health Legislation, which incorporates and respects differences in terms of gender, sexuality, ethnicity, religion, culture or disability.
- To manage a caseload of clients, in close collaboration with other members of the Multi-disciplinary team, within the CPA framework and to agreed local and national standards.
- To ensure that continuity of care is maintained and that appropriate and timely plans are made for the discharge of clients from the inpatient faculties ensuring Section 117 and CPA responsibilities are discharged.
- To provide Social Circumstances Reports for Mental Health Tribunals in line with Code of Practice and National Guidance
- To act as a Social Supervisor , providing high quality and timely reports to the Ministry of Justice as requested.
- To provide facilitation of groups including health promotion, wellbeing and those designed to address the offending needs of service users.
- To provide joint assessments and ongoing joint work with generic mental health teams where there is a forensic component to the service users presentation.
- To undertake assessment and intervention relating to dual diagnosis as an integrated part of service user treatment and care, supporting patients in minimising their drug and / or alcohol misuse and assisting them in gaining insight into the impact of using in respect of their mental health and offending behaviours.
- Ensure effective and timely review of all placements and out of area treatment packages in line with current performance management requirements,
- To ensure that the views and wishes of service users and cares are central to the care planning process and that their participation is maximised.
- To ensure that the needs of carers are fully assessed and that information and support is readily available.
- To ensure that the caseload work of the team is managed in accordance the revised Care Programme Approach (CPA) policy and that reviews are undertaken regularly and within agreed timescales. This will include robust crisis and contingency plans
- To work within Safeguarding Children and Safeguarding Adults Policies at all times, promoting the wellbeing of children and vulnerable adults and contributing to professional meetings as required
- To ensure that robust clinical assessment and management systems are in place and that all documentation including electronic records are up to date
- To ensure that the team delivers services which maximises the choice and independence of service users, promotes social inclusion and ensures that individualised budgets and direct payments are fully utilised by service users and carers.
- To ensure that effective links are developed with relevant organisational partners including statutory, voluntary and private providers as well as community groups and networks.
- To provide specialist forensic assessment and intervention to other Mental Health Teams and Criminal Justice Agencies as requested

- To provide OT assessments and intervention for clients within the team, applying specialist knowledge of Model of Human Occupation.
- To provide training and advice on lifestyle changes and adaptations to the clients social and physical environment
- To provide a comprehensive, culturally appropriate, evidence based assessments for clients to establish their occupational needs through standardised and specialist OT assessment tools.
- To act as a vocational lead within the team, ensuring that the occupational and employment needs are met.
In addition if Registered MH Nurse
- To provide and evaluate treatment interventions including medication management, physical observations and psycho-social interventions in line with NMC and Trust guidelines
- To provide monitoring for clients on Clozapine and to complete venepuncture training
- To ensure that interventions and practices are in line with infection control procedures.
- To ensure that all nursing interventions are in keeping with best practice and are evidence based.
In addition if AMHP:
- To carry out the functions of an Approved Mental Health Practitioner
- To participate in the Local Authority AMHP service and rota with an expectation that you will undertake a minimum of 12 Mental Health Act assessments per year of approval in order to maintain your warrant.
- To ensure that statutory responsibilities are undertaken in relation to the Mental Health Act, 1983 and other statutory legislation including the Children Act, 1989, 2004, the Human Rights Act, 1998, and the Mental Capacity Act, 2005
- To compile and complete statutory documentation, including an application for admission.
- To plan, negotiate and manage compulsory admission to hospital or arrangements for supervised community treatment orders, guardianship or alternatives to compulsion.
- To present information verbally and provide written reports at Mental Health Review Tribunal and any other legal hearing
