Job summary

This is a rare and exciting opportunity to join the Brent Home Treatment Team. We are seeking highly motivated, enthusiastic and clinically confident individuals, to compliment our highly skilled multidisciplinary team. You should have a genuine interest in working with acute mentally ill service users from a culturally diverse population. Successful individuals should have excellent communication and interpersonal skills, and be capable of problem solving and working well under pressure within a 24-hour, 7 days a week rota.

To be eligible to apply you will need evidence of continuing professional development and at least 2 years post registration experience.

You should have strong leadership skills and be able to manage a caseload of service users, co-ordinate the work of the team and act as supervisor for junior staff members. Successful candidates should be able to work well in a multi-disciplinary setting and be competent in developing strong links with other professionals. We can offer excellent group and individual supervision support and CPD opportunities. Learning and reflecting, as a group, is an important ongoing process for our service.

Main duties of the job

MAIN RESPONSIBILITIES:

The post holder will be involved in the ongoing development of the HTT, providing crisis response and acute home treatment to people with mental health problems at risk of admission to hospital. They will co-ordinate the care of service users of the HTT, and assist in the promotion of the use of evidence-based practice through the provision of clinical advice and support; acting as a role model; and providing education and training where appropriate.

Job responsibilities

Communication:

  1. To be fully conversant with statutory and non-statutory services in Brent, and promote effective liaison, working relationships and operational links as appropriate.
  2. To represent the HTT at sector, clinical and other meetings.
  3. To ensure safe and effective delivery of care to service users utilising the knowledge and skills of staff.
  4. To provide support, information and education to family members, carers and other professionals.
  5. To maintain a good public image of the HTT with any service user, visitor or carer by professional practice and conduct however difficult the circumstances.
  6. To undertake initial assessments of service users accepted by the HTT and take a lead role in the development and implementation of their individual treatment plans.
  7. To act as the named HTT worker for identified service users, ensuring the implementation of treatment plans, communication of significant clinical information within the team and to care co-ordinators, completion of documentation, clinical reviews and transfer/discharge procedures.
  8. To contribute to the gatekeeping of admissions and ensure effective throughput and discharge from the team.
  9. To demonstrate risk assessment and management skills, practising safely and according to individual service user needs.
  10. To liaise effectively with referrers and CMHTs, promoting good working relationships and continuity of clinical care.
  11. To screen and prioritise referrals to the HTT.
  12. To supervise junior staff as required.
  13. To act as a role model and clinically expert practitioner within the service.
  14. To demonstrate recognition that treatment takes place in peoples homes and to balance the need for engaging users with respect for this.

  1. To be available to receive clinical and managerial supervision.

Policy and/or Service Development:

  1. To participate in the formulation of policy and the development of the HTT.
  2. To assist in monitoring standards of clinical practice within the team and recommend changes where necessary.
  3. To participate in the implementation of professional and Trust-wide clinical policies and procedures.
  4. To maintain an effective working knowledge of the Mental Health Act 1983 and the Mental Health Act (Patients in the Community) 1995 and work within these legal requirements.

Management of Resources:

  1. To take assist in ensuring adequate staffing resources are available to provide the service.
  2. Assist in the recruitment and selection of staff.
  3. To assist in ensuring compliance with legal requirements concerning the prescribing, storage, transporting and administration of medicines.
  4. To work within a 24 hour rota system ensuring a comprehensive service to service users.
  5. To be aware of professional accountability and pay due regard to relevant professional codes of conduct and practice.
  6. To be responsible for the maintenance of accurate patient records.

Research and Development:

  1. To collect statistical and research data and compile reports as necessary.
  2. To participate in the evaluation of the service in conjunction with the Clinical Governance Department.
  3. To participate in research projects relevant to the team and its function.
