Horton Rehabilitation Services: There are 4 wards including 2 high dependency wards, a managed access ward and a community rehabilitation ward.

Main duties of the job
- To deliver an advanced comprehensive medicines optimisation service to patients and the multidisciplinary treatment team responsible for their care at Horton Rehabilitation Service
- Promote personalised care and shared decision making with patients, including supporting the move towards self-management of medication.
- Be a source of expert pharmaceutical advice to the MDT
- To provide leadership and support to the pharmacy team providing the Medicines Optimisation service at Preston House.
- Provide input into policy and procedures affecting the use of medicines at both local and trust wide levels.
