Job summary

Horton Rehabilitation Services: There are 4 wards including 2 high dependency wards, a managed access ward and a community rehabilitation ward.

Main duties of the job

  • To deliver an advanced comprehensive medicines optimisation service to patients and the multidisciplinary treatment team responsible for their care at Horton Rehabilitation Service
  • Promote personalised care and shared decision making with patients, including supporting the move towards self-management of medication.
  • Be a source of expert pharmaceutical advice to the MDT
  • To provide leadership and support to the pharmacy team providing the Medicines Optimisation service at Preston House.
  • Provide input into policy and procedures affecting the use of medicines at both local and trust wide levels.

EDUCATION AND QUALIFICATIONS

Essential

  • Vocational Masters Degree in Pharmacy (
  • Advanced Theoretical Knowledge acquired through Diploma (Higher Degree) level training
  • Member of the Royal Pharmaceutical Society

PREVIOUS EXPERIENCE

Essential

  • Demonstrate the ability to appropriately recommend, substantiate and communicate medicine related information to mental health patients, carers and clinical staff
  • Previous evaluated experience of mentorship and training skills
  • Monitoring and advising on drug expenditure
