OT in CNWL is very fortunate in having a long established professional structure and this includes an AHP director, Trust wide Professional Head of OT, borough and specialist Head OTs. Main duties of the job

We actively support the next generation of OTs through apprenticeships, student placements and the promotion of the profession as a career in schools. We are proud of the diversity in our workforce but have invested time in supporting and developing this further through our BAME OT group.

This is an exciting opportunity for one Band 6 Occupational Therapist to join the Hillingdon Primary Care Mental Health Team and work as part of the CCT in Hillingdon to develop this new model. The specific skills of OT are being sought to complement the existing nursing team. This will enable the post holder the opportunity to develop specific occupationally focused elements of the service including group work, vocation as well as holistic occupational needs assessments (40% of the role). You will provide care to patients to prevent escalation of their health and/or social care needs at a Primary care level thereby reducing the need for them to access secondary care services. Where appropriate you will work at the primary/secondary care interface to facilitate movement of patients between primary and secondary care services.

The generic element of the role (60%) includes providing mental health assessments and offers short term interventions to support this client group. The post holder will actively contribute to formulating a joint care plan with our CCT colleagues. You will also act as a contact point and provide a link/liaison role to GP practices.

In Hillingdon we are transforming our model of case management as part of the North West London - Integrated Care Model. It is an exciting opportunity to develop new ways of working to integrate physical health and mental health within primary care.. The means of delivering active case management and care coordination that has been developed in Hillingdon is the Care Connection Team. Initially introduced to address the care needs of patients over 65, with long term conditions and at risk of admission/frequent hospital attendance, the CCT is now expanding and evolving to include those aged 18 and over, as well as integrating mental health support.

The successful candidate will have the ability to effectively manage a rapidly changing caseload of individuals requiring time limited interventions. The role will also involve considerable travel around the Borough therefore requiring use of a motorised vehicle/bike/moped would be desirable but not essential. Most clinical work takes place within GP surgeries.

In return we offer a friendly, supportive working environment and demonstrate a commitment to learning and development for all staff. All OTs are provided with a professional supervision structure and access to the robust and well-developed OT CPD and R&D opportunities in the trust. The team currently operates Monday to Friday, 9-5pm but part-time and flexible working will also be considered.
