Job summary
This is an excellent opportunity to work as part of the liaison psychiatry team in Hillingdon Hospital, covering both wards and emergency department. It is a unique subspecialty in psychiatry where trainees gain a better understanding of the interplay between physical and mental health and the complexity of managing both hand in hand. The trainee will be supported by the wider multi-disciplinary team.
Main duties of the job
This role entails both new assessments and reviews of patients under the care of liaison psychiatry team on medical and surgical wards, as well as the Emergency Department. There will be active involvement in the daily MDTs where cases are discussed and management plans are formulated including signposting to different community services on discharge from hospital. There will be an opportunity to represent the team in family meetings as well as Care of the Elderly MDTs. The successful applicant will receive weekly clinical supervision by one of the consultants on the team and clinical supervision by the consultants as well as higher trainee on the team.
About us
The successful applicant will be well supported by the wider MDT and clinical supervisor. Daily handovers ensure optimal clinical governance, where thorough risk assessments are discussed, as well as quarterly review of serious incidents within the wider organisation. There is a 2-monthly reflective practice group, business meeting and weekly supervision with nominated clinical supervisor.
The Liaison Psychiatry team at Hillingdon hospital was established in 2012 and provides psychiatric input to the inpatient wards, Urgent Care Centre and A&E department of Hillingdon hospital, a busy 400+ bed District General Hospital. The team operates 24/7 and accepts all mental health and addictions referrals for patients aged 18 or above. There is a close working relationship with the Clinical Health Psychology service which is also based on the hospital site.

The CT1-3 will be responsible for contributing to the day to day assessment and management of referrals from all inpatient wards (including medical, surgical, obstetric, care of the elderly and neuro- rehabilitation) as well as the Urgent Care Centre and A&E department, under the supervision of Dr Habib and Dr Grewal.
General clinical duties of the post include:
- Seeing inpatients referred from wards as well as patients from the Urgent Care Centre and A&E Department at Hillingdon Hospital, either for initial assessments or ongoing review to provide diagnosis/formulation, develop management plans and facilitate discharge
- Undertaking comprehensive biopsychosocial mental health assessments and mental state examination for all patients seen, including risk assessment and consideration of issues such as mental capacity and safeguarding
- Working effectively as part of the Liaison Psychiatry MDT, and undertaking clinical work in conjunction with experienced mental health nurses.
- Participation in daily liaison MDT handovers
- Attendance of multi-disciplinary Care of the Elderly meetings at the Acute Trust to discuss and feedback on patients under the care of the liaison service
- Keeping accurate and contemporaneous clinical notes (both Hillingdon Hospital paper notes and CNWL electronic notes) as well as other relevant documentation e.g. risk assessments, discharge letters and handover or ward round summaries and correspondence with GPs
- Providing teaching for medical students, liaison service MDT and Acute Trust staff
- Undertaking audits, service evaluation or other small clinical projects suited to their level of training and this will be supervised by the consultant
- Liaison with the family, carers and staff of other services and organizations within the voluntary and statutory sectors advising on patient matters and facilitating discharge planning
- Interface with the staff providing out of hours psychiatry input at Hillingdon Hospital, to ensure adequate clinical handover at the beginning and end of each working day
- Offering prescribing advice to acute hospital colleagues (under the supervision of a senior clinician as needed)
- Undertaking WPBAs/SLEs in accordance with the requirements of their training programme
- CT1-3 will be expected to arrange cross cover for on-call shifts during periods of leave. However, daytime work within liaison will be covered by the rest of the team.
