Job summary
Working within a speciality team to manage the patient pathway, taking into consideration all aspects of that pathway to include: Tracking the progress of cancer patients through their pathway of care including through the diagnostic phase, pursuing results and ensuring that all results are managed appropriately to avoid potential delay. Patients may present through various routes and recording and tracking must be in accordance with National Cancer Waiting Times standards; Ensuring all patients receive appointments and next steps in their pathway in a timely fashion whilst overseeing any other aspect of managing the patient pathway; Maintain and update relevant databases as required. To support patients and their carers re-access specialist services without delay should they need to do so The post holder will work with the Colorectal Cancer team, specifically providing support to the Clinical Nurse Specialists, MDT Coordinators and Cancer Trackers to ensure timely care for all patients in relevant specialities. It is essential that the post holder exercise initiative commensurate with the role and that confidentiality is maintained at all times.
Main duties of the job
Interact with MDT Clinical Teams regarding urgent admissions, outpatient appointments and concerns expressed by patients/relatives and General Practitioners. Communicate with General Practitioners as to the appropriate course of action, i.e. admission or available clinic appointments.
Deal with enquires and problems encountered in the tracking of the patient pathway, communicating with all agencies, which includes taking the appropriate action escalating to the appropriate person as and when required.
Ensure timely return of investigations/reports requested by medical staff. Liaising with all staff concerned e.g. medical, nursing staff, Medical Records Department, X-ray Department, Patient Contact Centre, Pathology Department and Management etc.
Chase out-standing results and if necessary, liaise with outpatient booking teams to amend patient appointments, making sure follow-up appointments following investigations are arranged where necessary.
Assist the Clinical Nurse Specialists (CNS) in the coordination of the Colorectal Cancer service and other administrative aspects of the CNS role as and when required.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
