Job summary
We are seeking dedicated advanced clinical practitioners to join Barrow's new Integrated Wellness Service; an innovative programme, designed to transform care for patients athigh risk of hospital admission or with frequent hospital stays.
This is an opportunity to influence the development of a multidisciplinary team that brings together professionals from a range of services including (but not limited to) medicine, nursing, pharmacy, social care, therapy, voluntary sector, and community services.
Delivering complex case management within the community; the Wellness service provides a range of proactive and preventative interventions with some of the most vulnerable and complex patients living in Barrow community. The service facilitates multi-agency coordination and shared decision making via MDT meetings, as well as completing personalised care plans, comprehensive geriatric assessment of people who are frail and intensive treatment plans for long term condition management.
The service aims to reduce emergency admissions, promote independent living, and enhance coordinated care through a multi-disciplinary, holistic approach. The urgency is driven by the need to alleviate hospital pressures and improve patient outcomes
Main duties of the job
You will use the four pillars of advanced clinical practice to support your own development, the development of others, and the evolution of the service. You will work closely with Clinical Leaders, General Practitioners and Consultant Nurses both within the Wellness service and working across our Community, Cancer, Diagnostics and End of Life (CCDE) Division.
You will need to demonstrate effective and proven clinical leadership and advanced clinical decision making skills alongside an ability to build positive, productive relationships with teams and external stakeholders. You will be familiar with the current evidence base in assessment and management of long term conditions, frailty and community care.
To work at the level outlined in the HEE Multi-professional Framework for Advanced Clinical Practice (2017) with equal focus on all four pillars of advanced clinical practice: Clinical, Leadership/management, education, and research/audit
Provide expert practice, exercising a high degree of specialist professional judgement through the management of a complex caseload of patients recently discharged from hospital or at risk of readmission in the community.
About us
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
