Specialist Palliative Care Nurse - Community - South Lakes

Job summary

Working autonomously, you will have responsibility to assess, plan, implement and evaluate programmes of highly specialised evidence-based palliative care.

Integral to the post is the ability to demonstrate leadership skills, advanced interpersonal and communication skills alongside highly specialised clinical skills.

To plan and delivery education across a varied range of settings and to a wide range of health and social care professionals.

To support the development of the specialist palliative care team with service improvement, audit and research.

The teams are supported by medical staff and work closely with a comprehensive and well established Hospice at Home service and specialist hospice inpatient unit.

This is a full time post working initially with the Community team, based at Westmorland General Hospital, however will be flexible across UHMB. This is a permanent contract within the CDDE Division.

Main duties of the job

Lead specialist in palliative care.

Assess, plan, implement and evaluate programmes of evidence-based highly specialised advanced palliative care.

Provide expert, highly specialised advice, guidance and support to patients, carers/relatives, clinical practitioner and peers.

Provide expert evidence based practice, including expert assessment and non-medical prescribing.

Lead, devise and deliver specialist education and training to others.

Lead and work collaboratively and co-operatively with clinical colleagues to develop services and quality of care delivery.

Provide specialist expert leadership, knowledge, support and guidance to colleagues, patients and carers/relatives.

Undertake research and audit in the specialised area.

Support the education programme for specialist and end of life care.

About us

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
