Job summary
An exciting opportunity has arisen for a knowledgeable nurse with at least 2-years cardiac experience to join our dynamic cardiac rehabilitation team. The role will based at Westmorland General Hospital in Kendal, but travel between sites at Lancaster and Barrow-in-Furness is required. Access to timely travel between sites is essential.
The post holder will be a key member of the multi-disciplinary team in delivering a comprehensive cardiac rehabilitation programme for patients with cardiac conditions and heart failure.
The post holder will actively assist in the management of the service and will conduct patient assessments at sites across the trust area and will be involved in assessing patients for exercise and monitoring the exercise programmes with the exercise physiologist.
Main duties of the job
The post holder will provide an autonomous nursing service by attending to referrals from inpatient multi-disciplinary colleagues.
The post holder will be expected to provide individual advice, group work and telephone/virtual support to patients and their carers. Provision of evidence based secondary prevention information will be required involving patients and their relatives.
The post involves liaising with professional colleagues in other disciplines and with statutory and voluntary organisations.
About us
We operate from three main hospitals -Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
