Discharge to Assess

Specialist Occupational Therapist Band 6

We have a exciting opportunity for a Specialist Occupational Therapist to join our Discharge to Assess Team at Royal Lancaster Infirmary. The team works to deliver a Discharge to Assess service providing therapy based assessment and treatment.

The Discharge to Assess service is well established across our organisation and we are expanding our services to cover 7 days a week. The post is based at the Royal Lancaster Infirmary and is part of our cross bay Discharge to assess team covering discharges to the Lancashire, South Lakes and Furness area . The service provides assessment and rehabilitation for patients on discharge to provide a holistic assessment within the person's home. This includes being able tosupport and identify discharge requirements and making referrals on to Health and Social care community services.

All assessments will be within the patient's home at point of discharge from hospital.

Main duties of the job

You will have broad previous experience within an acute inpatient, admission avoidance or community-based rehabilitation services.

You will have knowledge and experience in the assessment, treatment and goal planning with patients presenting with varied physical conditions and will be motivated to join us in improving our patients' experiences and outcomes.

Requirements:-

A Diploma or Degree in Occupational Therapy approved by the Royal College of Occupational Therapists

Registration with HCPC as an Occupational Therapist

The ability to travel to a variety of venues throughout the community to deliver patient assessments within an identified locality and to attend meetings

In addition to a comprehensive induction package we offer Team support, staff supervision and in-service training opportunities

About us

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

Education and Qualification

Essential

  • Health and Care Professions Council registered Occupational Therapist
  • Diploma or Degree in Occupational Therapy approved by the Royal College of OT, UK
  • Post graduate training or experience to post graduate diploma level or equivalent

Desirable

  • Computer qualification e.g. European Computer Driving License (ECDL)
  • Accredited training in risk assessment and risk management
  • Practice placement educator accreditation
  • Membership of the British Association of Occupational Therapists

Experience

Essential

  • Relevant post registration experience as an OT to achieve knowledge and skills applicable to this post
  • Post registration experience in a relevant clinical area

Desirable

  • Experience of research or audit
  • Experience as a supervisor of OT (Band 5) and/or OT support staff and/or OT students
