Job summary

This is an exciting opportunity for an experienced pharmacist to utilise clinical independent prescribing and leadership skills as they develop and lead clinical an operational pharmacy service provided to patients on the virtual wards and the urgent community response service in Tees Community.

Hospital at Home service provides hospital level care for patients in their own home to avoid hospital admission and facilitate discharge. Urgent community response reviews patients within 2 hours of referral to avoid hospital admission.

Hospital at Home offers a hospital standard of community care in people's own homes for frailty patients - who are at highest risk of poor health outcomes such as falls, disability, admission to hospital, or the need for long-term care.

The service helps to reduce avoidable hospital admissions, while ensuring patients get the care they need from the comfort of their own homes.

Patients are monitored daily by a multidisciplinary team and are overseen by a GP or consultant. Visits from the team are arranged as and when required to meet individual needs.

This is an excellent opportunity for a pharmacist to be a crucial member of the MDT for one of the most rapidly expanding clinical services within the Tees Valley Community.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Main duties of the job

We are looking to appoint a confident and competent pharmacist who can demonstrate effective clinical skills and service development. You will be responsible for leading and developing the clinical pharmacy service within the Urgent Community Response service (UCR) encompassing the

Hospital at Home Virtual Ward to provide excellent at home and on site patient centered care.

The role will additionally involve leading and implementing service improvements and expansion. The successful candidate will be provided with all the relevant training and support, as well as provided with ample opportunities to allow personal growth, and career progression. You will work

as a crucial part of the multi-disciplinary team working alongside consultant, GP, nursing and pharmacy colleagues to deliver safe and effective patient care.

The pharmacy department at South Tees hosts over 180 staff, and provides a stimulating, friendly, and professionally rewarding environment for staff to develop their clinical expertise and professionalism.

Essential

  • Qualified to a master's degree level (MPharm or equivalent)
  • Registration with the General Pharmaceutical council or Great Britain as a practising pharmacist
  • Non-medical prescriber qualification
  • Post graduate Diploma in clinical pharmacy, or equivalent

Experience

  • Teaching, tutoring, or mentoring of pre-registration pharmacists
  • Worked as a clinical pharmacist at a high level
  • Experience and involvement of service development

Desirable

  • Experience of a pharmacist led clinic
