Job summary
An exciting role has opened to work as a Community Clinical Support Worker Higher Level within the multidisciplinary team working across Middlesbrough, Redcar and Cleveland. The post holder will be predominantly based in Redcar Coastal PCN.
This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Middlesbrough, Redcar & Cleveland.
To work as part of the community nursing team; we deliver a 7 day service , including some weekends.
The post holder will undertake a broad range of care activities, including wound care, insulin administration and venepuncture
The role will require some flexibility and this may require the post holder to work across acute and community in response to service demands.
To support the delivery of patient care and other duties that contributes to the care environment. This might include, for example, having a "Link" role; supporting therapy and nursing/medical staff.
To support the delivery of a range of duties that support patient care and maintain a suitable working environment.
Successful candidate must also have access to a vehicle for work purposes to meet the requirements of the role.
Main duties of the job
The post holder will be an active member of the multi-disciplinary team (MDT), undertaking personal care activities, delegated clinical care duties, including clinical observations as directed by registered professionals and within clearly defined parameters, to provide healthcare and support that is individualised to the patient needs.
The post holder will update and maintain accurate records of any care provision and update and escalate any concerns from their observations and activities to the appropriate registered professional.
The post holder has a personal responsibility and duty of care to patients/service users; they are expected to comply fully with all Trust policies and protocols, including those related to patient safety and the prevention of healthcare associated infections. They must also promote dignity, privacy, and respect, to deliver compassionate, safe, high quality patient care.
