Staff Nurse Neonatal Outreach

Job summary

The neonatal outreach team is a small and friendly team who work between the neonatal unit and the community, ensuring that parents are supported to take their children home when additional input is required.

They require a motivated and dynamic band 5 nurse to join their ranks.

They will work closely with our larger (and equally friendly) neonatal team taking part in improving the discharge process.

University Hospital Tees is an excellent organisation to work for and there are many supportive and educational opportunities to ensure that staff become well rounded members of the team.

The postholder will work as part of the 7-day Neonatal Outreach Team and will provide a high standard of nursing care for neonates and their families preparing for discharge from NICU as well as supporting them at home following discharge. This will include planning for discharge with the multidisciplinary team as well as preparing the family beforehand with teaching and information.

There will be time spent on the NICU making sure that families are aware of the home service and that staff are sited on moving families forward with appropriate parentcraft and education, discharge is crucial to flow through the NICU. This may also include families who are receiving care on the Transitional Care Unit.

Time will also be spent with the community team and caring for families at home.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role

Main duties of the job

This post will also be integral to supporting new initiatives such as home phototherapy and antibiotics into the future.

To facilitate the timely and safe discharge of neonates into the care of the Neonatal Outreach team, supporting ward staff to manage this in a well-timed manner.

To ensure that families are well prepared for going home and that they have been exposed to all necessary training and are aware of what will happen whilst at home. This will be done mainly by the ward staff but will be supported and driven by this post.

To ensure that Ficare principles are included in this and that families are part of the process including the decision making and when arranging dates for deliveries and discharge.

To liaise with the multidisciplinary team in relation to the discharge and follow up of patients.

Organise discharges from the NICU to ward 21 under the paediatric transitions policy.

To undertake home visits and support families to receive care at home ensuring that they are confident and comfortable managing therapies at home as required i.e. oxygen, tube feeding, medicine administration (including NAS management).

To participate in community clinics as appropriate to role.

To act as an advocate for families.
