Job summary
An excellent opportunity has arisen for an Advanced Clinical Practitioner (ACP) within the Urgent Community Response (UCR) and Hospital at Home Virtual Ward (VW) service within Redcar and Cleveland and Middlesbrough.
Are you a clinical expert who puts the clinical and holistic needs of our patients first to ensure safe, high quality and effective care? Are you a passionate Advanced Clinical Practitioner with expert knowledge of urgent community services, virtual wards or same day emergency care services? Would you like to work in advancing services in the community?
We are looking for experienced and highly skilled clinicians to develop and work within the advancing services of UCR and VW. The post holder will be working autonomously as an Advanced Clinical Practitioner within the UCR and Hospital at Home VW service in Redcar and Cleveland and Middlesbrough.
Please note consideration will be made for applicants who do not meet the full criteria of the role, who have considerable experience, who already hold level 7 Non-Medical Prescribing and level 7 Advanced Physical Assessment Skills Qualification.
This would be a band 7 clinical practitioner post with the transfer to band 8a ACP role on completion of the full qualification under annex 21 (please view both job descriptions and person specifications)
Main duties of the job
This role will be at an advanced clinical level in the direct delivery of high-quality care making critical clinical decisions based on advanced clinical skills assessment within the community setting. Working effectively across all four pillars of advanced clinical practice as set out by Health Education England (2017).
The post holder will be able to evidence all four pillars of the advanced clinical practice framework and benchmark to advanced clinical practice levels across all four pillars.
The post holder will provide clinical leadership and support within the nursing/AHP teams in developing the service to improve the patient experience, supporting Consultants/ GP with specialist interest in Frailty with day-to-day clinical activity (to be determined according to need of specialist area).
Providing in-patient, same day emergency care, outpatient and community areas with consistent high quality clinical care, supporting trust management teams with on-going clinical care provision and assist with effective patient flow through the trust.
You will be/have:
- Registered healthcare practitioner
- Independent non-medical prescribing (where legally permitted)
- Portfolio demonstrating continued professional development and ongoing assessments
- ILS/ALS provider
- Formal research module completed at level 7 (master's level).

The current UCR and Hospital at Home have a well-established workforce of Clinical Practitioners, Community UCR Sisters, Senior Occupational Therapists, Senior Physiotherapists, Community Staff Nurses and Health Care Support workers who provide a wraparound service for our patients who acutely deteriorate in the community.

After you complete these four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3,5 and 7
