Job summary

An Excellent opportunity has arisen within Friarage Hospital Patient Flow Team.

We are seeking to appoint an enthusiastic, dynamic, and well organised person to work as a patient flow coordinator at the Friarage hospital.

Dedicated to delivering a high standard of care you will be able to demonstrate excellent interpersonal and communication skills and develop your skills and knowledge.

You will have excellent communication skills, be able to coordinate and collate real time information on current and predicted bed availability and facilitate the effective use of acute Friarage Hospital bed and Rehabilitation bed capacity.

You will work as part of the multidisciplinary team and use your own initiative and judgement. This role is central to the flow of patients within the Friarage.

Main duties of the job

  • To work collaboratively within the collaborative supporting with patient flow and discharge planning
  • Collate and provide required information and updates relating to the collaborative bed bases to the site sister / discharge facilitator as appropriate.
  • Assist the discharge facilitator to ensure the accuracy of the MIYA / E-Camis system.
  • Assist with the booking and managing of transport services (in house and ambulance) required for effective patient discharges.
  • Communicate and liaise with internal and external support services to ensure effective flow throughout the collaborative.

  • NVQ Level 4 in healthcare, Customer Service or Business and Administration or equivalent/equivalent level of knowledge and skills
  • GCSE Grade C/4 or higher in English

Desirable

  • Educated to degree level

  • Competent with software programmes including Micosoft Word, Outlook, Excel and Power Point
  • Knowledge and experience of providing person centred care in a health and social care environment.
