Job summary
South Tees Foundation Trust Pharmacy Department is proud to advertise this new post of Advanced Clinical Pharmacist for Diabetes & Endocrinology. This new role is part of the progress and expansion of our advanced level of practice for pharmacists.
The post-holder will be required to innovatively develop the role, and work on their own initiative. They will work closely with the Diabetes & Endocrinology multi-disciplinary team (MDT) and the wider pharmacy team, ensuring that services are provided in accordance with relevant professional, ethical, legal and locally agreed policies, procedures, guidelines and standards. The post-holder will be required to develop excellent rapport with patients as well as the MDT. Exceptional communication skills are vital for this role.
The post-holder will be a key member of the clinical pharmacy team, making major contributions to the development and delivery of pharmacy medicines management strategies and the implementation of development plans for pharmacy services

Main duties of the job
The post holder will deliver safe, effective, and optimum clinical pharmacy services to Diabetes & Endocrinology patients at South Tees Hospitals, in addition to providing expert advice, guidance and information on treatment choices, medicines use, clinical pharmacy and financial issues to senior staff and mangers within the services.
The post will involve development of clinical pharmacy services in Diabetes & Endocrinology to ensure medicines optimisation. The postholder will actively consult patients, and make clinical interventions to improve patient care and patient safety. The post holder will also continuously complete service evaluation and monitoring of KPIs to continuously make improvements to the service.
