This exciting new role has been developed to support the new Urgent Community Response (UCR) multidisciplinary team working across Hambleton and Richmondshire.

This is an opportunity for anyone interested in working alongside nursing and therapy to help bridge the gap between hospital and home. The UCR Team will identify patients on the wards at The Friarage Hospital that can be assessed in their own home to facilitate early discharge. You will provide both rehab and care following a person-centered plan within the patients' own home post discharge.
The role will require some flexibility and this may require the post holder to work across acute and community in response to service demands and therefore you would need to be able to travel independently throughout the area.
Main duties of the job
To provide a 24 hour service where the post holder may well be working in a patients home overnight.
To work as part of the Integrated Community Urgent Community Response (UCR) therapy/nursing team to deliver a 7 day, 24 hour same day UCR service based in the community, to support discharge to assess and admission avoidance. To work flexibly to meet the changing needs of the patients.
The Healthcare Support Worker is a member of the team who will support patients at home during a 72 hour period of crisis until a care package can be instigated.
The post holder will undertake a broad range of care activities, including personal care, sitting service under the direct supervision of a Registered Practitioner.
Each person that you work with will have individual needs, but the kind of support you might give includes:
- Assistance with washing and dressing, getting in/out of bed on/off the toilet.
- Prompting with medication
- Base line observations
