Senior Integrated Support Assistant (Home First)

Up to 37.5 Hours per week (Permanent) - 7 day service (shift patterns apply)

This exciting role has been developed to support the 'home first' multidisciplinary team working across the South Tees locality.

You will need to be able to work on your own; as part of a team and be involved with individuals with a range of medical, social and mental health conditions, therefore knowledge and experience of this is needed. You will work within agreed protocols and carry out therapeutic interventions and care programmes under the supervision of qualified therapists and nurses.

Main duties of the job

This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Middlesbrough, Redcar & Cleveland.

South Tees Hospitals NHS Foundation Trust is one of the largest teaching hospital trusts in the country, close to the North Yorkshire Moors and within easy reach of Newcastle and Leeds. It comprises two acute hospitals, The James Cook University Hospital and Friarage Hospital Northallerton as well as established community service teams across Middlesbrough, Redcar & Cleveland, Hambleton and Richmondshire localities.

Excellent communication skills, organisational skills, enthusiasm, ability to travel around the locality, commitment and ability to work with a wide range of agencies are also vital in this post.

This is an opportunity for anyone with previous experience working within nursing, therapy or in care settings to help bridge the gap between hospital and home. As a senior integrated support assistant you will provide both rehabilitation and nursing care following a person centred plan within the patients' own home.

About us

After you complete this one and a half day of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses and bitesize programmes and leadership apprenticeships at level 3,5 and 7
