Job summary
The post is based within the Middlesbrough Mobile Rehabilitation Unit, which consists of Physiotherapists, Occupational Therapists, Therapy Assistant Practitioners and Therapy Assistants.
Patients are treated in care homes to facilitate discharge home following a period of time in hospital. The team aim to optimise their rehabilitation potential and encourage independence.

Main duties of the job
Assessment and treatment of patients in care homes to facilitate discharge home.
MDT working, including patients family.
Liaise with colleagues in Health and Social Care and Voluntary Agencies.
