Job summary
This new and exciting role is created to provide additional specialised advice and guidance to ensure equity of care for people with a learning disability across South Tees Hospitals.
We are looking to recruit a qualified and experienced Learning Disability Registered Nurse to join South Tees Hospitals. The successful candidate will be able to demonstrate they have the necessary skills and knowledge to ensure patients, across all ages, have a positive experience throughout their journey at South Tees Hospitals. The role involves supporting patients with a learning disability to have full and appropriate access to healthcare across the service. The role will involve working with multiagency colleagues, informal carers, and to be an advocate for the needs of patients with a learning disability.
The post holder will require the knowledge, skills and capability to be able to work as part of a team, or autonomously, supporting complex patients. The post holder will have the knowledge and experience to be able to provide support/advice and specialist training, working across all sites in the Trust. The successful candidate may need to work flexibly on occasions.
Main duties of the job
- The primary purpose of the role is to ensure that patients with a learning disability who need to access acute health care services receive the highest possible standard of care.
- Provide specialist support and advice to the multidisciplinary team in the management of patients with a learning disability.
- Ensuring that systems and processes are in place to identify patients with a learning disability and that their pathways of care are reasonably adjusted to meet their needs.
- Ensuring the availability of a comprehensive range of patient information, designed for and with patients with a learning disability.
- Ensuring that families and informal/professional carers of patients with a learning disability are properly supported informed and engaged.
- Develop and deliver effective training in relation to learning disabilities.
- Complete audits to assess and identify potential improvements in the care of patients who have a learning disability.
