30 hours per week, Monday - Friday 8.30-4:30pm, days and times to be negotiated to suit successful candidate and service.

Countywide, based at the Independent Living Centre in Cheltenham.

This exciting post, based in Cheltenham, involves working closely with the Stroke Early Supported Discharge service and the Community Neurology Service supporting stroke survivors to adjust to life after stroke.

This exciting role in primary care is open to nurses and therapists with experience in working with stroke patients.

This role involves visiting stroke patients in their own homes, assessing their needs post stroke, and providing information and support to patients and their carers in accordance with the local and national guidelines. There is a strong emphasis on health promotion and self-management, as well as referring on to appropriate services and signposting to voluntary sector organisations.

Experience in stroke care and multidisciplinary working are essential as well as excellent communication and problem solving skills. The successful candidate will be an autonomous practitioner with good self-management skills.

A driving licence and access to a car are essential.

Main duties of the job

The Stroke Coordinator will assess, review and coordinate aftercare for stroke survivors and their relatives/carers. The post holder will independently manage a clinical caseload which coordinates and supports the patient journey through the community rehabilitation setting and beyond.

The post holder will provide high quality patient care in line with National and Local standards and Trust guidelines. This will include a multi-agency approach and patient centred care. The post holder will also contribute to national and local audits and research.

The post-holder has no budgetary responsibility and is expected to have day to day operational and supervisory responsibility for more junior staff and students.
