We are looking for an OT to come and join us in our Integrated Community Team, here in the Cotswolds.
We are a team comprising of OT's, Physio's, Community Nurses and Home First all working to promote independence for our population in the Cotswolds. This post is based in Bourton on the Water and will mainly cover the North Cotswolds, working alongside our colleagues in Adult social care, primary care, the voluntary sector and the acute services.
We are particularly looking for an OT who has skills in Rehabilitation progressing through to minor and major adaptations utilising the Disabled facilities grant process as required. This is a unique opportunity to work for an NHS organisation who also provides long term major adaptations, removing disabling barriers from the home environment to promote independence.

Main duties of the job
To work as a Senior Occupational Therapist in the Integrated Community Team (ICT). To work with adults with physical disabilities, cognitive impairment, mental health issues and complex social and community needs, experiencing occupational performance difficulties within and around their home environment.
To work as an integral member of the ICT to provide a service through assessing, collaborative goal setting, delivering and evaluating treatment To use a recognised model of practice to underpin all practice Assess and treat own caseload of patients/clients, adopting a health coaching approach for all intervention The post holder will provide supervision and support to registered OTs, OT assistants and reablement colleagues as appropriate.
Provide occupational therapy advice to other disciplines
May participate in departmental research

- To monitor, evaluate and modify goals/treatment using appropriate outcome measures to ensure effectiveness of interventions.
- To be competent in environmental risk assessments and managing risk autonomously within the home environment.
- To promote a positive risk taking approach to facilitate retention of patient independence and well-being.
- To manage clinical risk within own patient caseload. This may involve complex decision making regarding patients being safe to remain in their place of residence or requiring admission to hospital.
- To assess patients capacity, gain valid informed consent to treatment and where such capacity is lacking/absent, to work within a legal framework to manage the patient appropriately.
- To provide spontaneous and planned advice and instruction to relatives, carers, other disciplines and agencies to promote understanding of the aims of occupational therapyfor each individual patient, and to ensure continuation of the individualised goal plan that has been set in collaboration with the patient/family/carer. This will include moving andhandling advice and falls prevention.
- To attend and initiate multidisciplinary / multi agency team meetings and case conferences to ensure the coordination of patient care. This will include review of patient progress, setting of short and long-term goals, and discharge planning
